摘　　要：目的:探讨主动式社区治疗模式(Assertive Community Treatment,ACT)对社区精神分裂症患者的干预效果。方法:对100例干预组患者进行ACT模式干预,100例对照组患者进行常规社区干预,分别在入组时,第3,6,9,12个月评估阳性与阴性症状量表(PANSS),个人和社会功能量表(PSP),自尊量表(SES),精神分裂症患者生活质量量表(SQLS),统计再入院次数和住院天数,比较两组间的差异。结果:重复测量方差分析结果显示干预组的PANSS量表各因子得分、PSP量表评分、SES量表评分时间主效应有显著的统计学意义(P<0.01),生活质量量表的精力及动力因子时间主效应显著,有统计学意义(P<0.05)。PSP量表评分、SES量表评分组别主效应有统计学意义(P<0.05)。干预组再次入院平均住院天数较对照组减少,差异有统计学意义。结论:主动式社区治疗模式能够改善社区患者的精神症状,提高自我照料技能,改善其社会关系,提高自尊体验,缩短再次入院的住院天数。Objective:To explore the effect of Assertive Community treatment model(ACT)on the intervention of community schizophrenics.Method:100 patients in the intervention group were treated with ACT mode intervention,and 100 patients in the control group were treated with routine community intervention,Positive and negative symptom scales(PANSS),Personal and Social Functioning Scale(PSP),Self-Esteem Scale(SES),Quality of Life Scale(SQLS)for schizophrenics were assessed at baseline and at 3,6,9 and 12 months respectively.Number of readmissions and length of stay were counted.Results:Repeated-measurement analysis of variance results showed that the differences in time effect of each factor in the PANSS scale,PSP scale and SES scale of the intervention group were statistically significant(P<0.01),while the time effect of energy and dynamic factor in SQLS was significantly different(P<0.05).The main effect of each factor score in the PSP scale and SES scale were significantly different(P<0.05).The average length of re-hospitalization in the intervention group was shorter compared with control group,and the difference was statistically significant.Conclusion:The ACT model can improve negative symptoms,enhance self-care skills,modify their social relations,enhance self-esteem experience and shorten the average days of re-hospitalization in community schizophrenic patients.
