摘　　要：目的:探讨Ilizarov环形外固定架骨延长技术治疗儿童尺骨骨干续连症的方法及其疗效。方法:回顾性分析了2015年9月至2018年11月郑州大学第一附属医院小儿外科收治的7例尺骨骨干续连症患儿临床资料(男5例,女2例),共7臂畸形,年龄8.51~13.0岁。均采用尺骨截骨结合Ilizarov环形外固定架逐步延长尺骨的方法治疗。术后定期随访,末次随访时评价患肢畸形及功能改善情况。结果:7例患儿均获得随访,随访时间12~50个月,尺骨延长长度2.3~4.5 cm,桡骨头有效复位。术后X射线评价和功能评价指标均优于术前(P<0.05)。1例患儿出现骨延迟愈合,调整延长速度后逐渐愈合;1例患儿出现针道感染,给予换药加口服抗生素治疗后治愈。无血管、神经损伤和取出外固定后再发骨折等并发症。结论:Ilizarov环形外固定架骨延长技术治疗儿童尺骨骨干续连症可有效改善患肢功能及外观,疗效满意。Aim:To investigate the approach and effects of Ilizarov ring external fixator bone extension technique in treating ulnar diaphyseal aclasis in children.Methods:A retrospective analysis was performed on the clinical data of 7 children with ulnar diaphyseal aclasis admitted to the Department of Pediatric Surgery of the First Affiliated Hospital of Zhengzhou University from September 2015 to November 2018,including 5 males and 2 females with a total of 7 deformed arms,aged(8.5-13.0)years.Ulna osteotomy combined with Ilizarov ring external fixator was used to gradually lengthen the ulna.The patients were followed up regularly and the malformation and functional improvement were evaluated at the last follow-up.Results:All 7 children were followed up,during the follow-up period of 12-50 months,the ulna was lengthened,the length was 2.3-4.5 cm,and the radius head was effectively reset.The indexes of postoperative X-ray evaluation and functional evaluation were better than those before surgery(P<0.05).Delayed bone healing was observed in 1 patient,and the bone gradually healed after adjusting the extended speed.One child developed needle infection,who was cured by dressing change and oral antibiotics.There was no vascular,nerve injury,or complications such as fracture occurred after external fixation.Conclusion:Ilizarov ring external fixator bone extension technique can effectively improve the function and appearance of the affected limb in the treatment of ulnar diaphyseal aclasis in children.
