丁苯酞软胶囊治疗帕金森病痴呆疗效和安全性的Meta分析

摘　　要：目的通过Meta分析评价丁苯酞软胶囊治疗帕金森病痴呆(Parkinson disease dementia,PDD)的疗效和安全性。方法计算机检索中国知网、万方、维普、中国生物医学文献数据库、PubMed、EMbase、Wed of Science和The Cochrane Library数据库中关于丁苯酞软胶囊治疗PDD的随机对照试验,检索时限为2002年1月1日至2020年4月30日。根据纳入及排除标准,筛选文献、提取资料并评价文献质量,采用RevMan 5.3软件进行Meta分析。结果最终纳入12项研究,共989例PDD患者。与对照组相比,丁苯酞软胶囊治疗组在提高简易精神状态评价量表(Mini-mental state examination,MMSE)评分[SMD=0.87,95%CI(0.61,1.14),P<0.01]、蒙特利尔认知评估量表(Montreal cognitive assessment,MoCA)评分[SMD=0.89,95%CI(0.71,1.08),P<0.01]、日常生活能力量表(Activities of daily living,ADL)评分[SMD=1.82,95%CI(1.27,2.38),P<0.01]、总有效率[RR=1.25,95%CI(1.16,1.34),P<0.01]和降低帕金森病评定量表(Unified parkinson disease rating scale,UPDRS)评分[SMD=-3.05,95%CI(-3.31,-2.79),P<0.01]方面更有优势,但两组在不良反应发生率方面无差异[RR=1.57,95%CI(0.54,4.54),P=0.41]。结论丁苯酞软胶囊治疗PDD患者具有改善认知功能、提高总有效率、改善帕金森症状及日常生活能力的积极作用,且安全性良好。Objective To systematically assess the efficacy and safety of butylphthalide soft capsule in the treatment of patients with PDD(Parkinson disease dementia,PDD).Methods Randomized controlled trials of butylphthalide soft capsule for treating patients with PDD were electronically searched from CNKI,Wan Fang Data,VIP,CBM,PubMed,EMbase,Wed of Science,The Cochrane Library from January 1,2002 to April 30,2020.According to the inclusion and exclusion criteria,two investigators independently screened literature,extracted data and assessed the quality of the literature.Meta-analysis was conducted by using RevMan 5.3 software.Results Twelve randomized controlled trials with a total of 989 patients with PDD were included.Compared with the control group,the butylphthalide soft capsule treatment group improved the MMSE score[SMD=0.87,95%CI(0.61,1.14),P<0.01],the MoCA score[SMD=0.89,95%CI(0.71,1.08),P<0.01],the ADL score[SMD=1.82,95%CI(1.27,2.38),P<0.01],total effective rate[RR=1.25,95%CI(1.16,1.34),P<0.01]and decreased the UPDRS score[SMD=-3.05,95%CI(-3.31,-2.79),P<0.01]of patients with PDD.However,there was no statistical difference in the number of adverse events between the treatment and control groups[RR=1.57,95%CI(0.54,4.54),P=0.41].Conclusion Butylphthalide soft capsules have positive effects on improving cognitive function,improving total effective rate,improving the symptom of Parkinson disease and daily life ability in patients with PDD.No serious adverse events have been observed.
