彩色多普勒超声联合血清学肿瘤标志物对卵巢良恶性肿瘤的鉴别诊断价值

摘　　要：目的探讨彩色多普勒超声联合血清糖类抗原125(CA125)、糖类抗原724(CA724)检测对卵巢良恶性肿瘤的鉴别诊断价值。方法111例卵巢肿瘤患者均接受血清CA125、CA724检测和彩色多普勒超声检查。比较血清CA125、CA724检测和彩色多普勒超声单独及联合检测诊断卵巢良恶性肿瘤的灵敏度、准确度和特异度;并分析术前、术后7天,卵巢良恶性肿瘤患者CA125、CA724水平、搏动指数(PI)及阻力指数(RI)。结果病理学诊断结果显示,111例卵巢肿瘤患者中,良性57例,恶性54例。CA125+CA724+彩色多普勒超声检查诊断卵巢良恶性肿瘤的灵敏度和准确度均高于单独血清CA125检测、CA724检测及彩色多普勒超声检查,差异均有统计学意义(P﹤0.05)。术前,卵巢良性肿瘤患者CA125、CA724水平均低于卵巢恶性肿瘤患者,PI及RI值均高于卵巢恶性肿瘤患者,差异均有统计学意义(P﹤0.05)。术后7天,卵巢良恶性肿瘤患者CA125、CA724水平均低于本组术前,且卵巢良性肿瘤患者CA125、CA724水平均低于恶性肿瘤,差异均有统计学意义(P﹤0.05)。术后7天,卵巢恶性肿瘤患者RI和PI值、卵巢良性肿瘤患者RI值均高于本组术前,差异均有统计学意义(P﹤0.05);但卵巢良性肿瘤患者PI值与术前比较,卵巢良恶性肿瘤PI和RI值比较,差异均无统计学意义(P﹥0.05)。结论CA125、CA724及彩色多普勒超声检查均可有效鉴别卵巢肿瘤,但三者联合联合检测可提高诊断准确度、特异度,且CA125、CA724可有效评估临床疗效,为其后续治疗提供可靠的指导意见。Objective To investigate the value of color Doppler ultrasonography combined with serum carbohydrate antigen 125(CA125)and carbohydrate antigen 724(CA724)in the differential diagnosis of female ovarian benign and malignant tumors.Method 111 patients with ovarian tumors all were all tested for serum CA125,CA724 detection and color Doppler ultrasonography.The sensitivity,accuracy,and specificity of serum CA125,CA724,color Doppler ultrasonography,and combined tests of them in the diagnosis of ovarian tumors were compared.The levels of CA125,CA724,pulsatility index(PI)and resistance index(RI)between benign ovarian and malignant tumors were analyzed before operation and 7 days after operation.Result Pathological examination showed among 111 patients with ovarian tumors,there were 54 cases with malignant ovarian tumors and 57 cases with benign ovarian tumors.The sensitivity and accuracy of CA125+CA724+color Doppler ultrasonography in diagnosing benign and malignant ovarian tumors were higher than those of serum CA125,CA724 and color Doppler ultrasonography alone,and the differences were statistically significant(P<0.05).Before surgery,the levels of CA125 and CA724 in patients with benign ovarian tumors were lower than those with malignant ovarian tumors,and the values of PI and RI were higher than those with malignant ovarian tumors,and the differences were statistically significant(P<0.05).7 days after operation,the levels of CA125 and CA724 in patients with benign and malignant ovarian tumors were lower than those before surgery,and the levels of CA125 and CA724 in patients with benign ovarian tumors were lower than those in malignant tumors,the differences were statistically significant(P<0.05).7 days after operation,the PI values of patients with ovarian malignant tumors and the RI values of patients with ovarian benign tumors were higher than those of this group before operation,and the difference was statistically significant(P<0.05).However,there was no statistically significant difference in in PI values betwee
