车祸创伤暴露者PTSD症状与记忆功能变化的相关性研究

摘　　要：目的:探讨创伤后应激障碍(PTSD)患者临床症状与记忆功能的相关性。方法:纳入车祸创伤所致PTSD的患者(PTSD组)及健康体检者(对照组)各30例,均行PTSD症状评估量表(PCL-5)和韦氏记忆量表中文修订版(WMS-RC)评估。结果:PTSD组WMS-RC中各记忆功能评分均低于对照组(t=-2.850,Z=-3.434,Z=-3.572,Z=-2.897,t=-2.721,Z=-3.241,Z=-3.434,均P<0.05);PTSD组PCL-5总分及4个症状群评分均高于对照组(Z值分别为-6.736、-6.924、-6.668、-6.856、-6.771,均P<0.05);行Pearson相关分析显示,PTSD组PCL-5总分与WMS-RC的7个记忆功能分测验评分均呈显著负相关(P<0.05),闯入症状与除视觉再生外的各记忆功能均呈负相关(P<0.05),回避症状与除视觉再认、视觉再生及触觉记忆之外的各记忆功能呈负相关(P<0.05),负性心境改变、高警觉性症状与各记忆功能评分均呈显著负相关(P<0.05)。结论:PTSD患者存在记忆功能损伤,且随着PTSD症状加重,记忆功能的损伤也越明显;其中负性心境改变、高警觉性两大症状群对各记忆功能的损伤有显著影响。Objective:To investigate the correlation between clinical symptoms and memory function in patients with post-traumatic stress disorder(PTSD).Methods:30 patients with PTSD(PTSD group)caused by car accident trauma and 30 healthy peoples(control group)were assessed by PTSD symptom assessment scale(PCL-5)and Chinese Wechsler memory scale(WMS-RC).Results:The scores of memory function in WMS-RC of PTSD group were lower than those of control group(t=-2.850,Z=-3.434,Z=-3.572,Z=-2.897,t=-2.721,Z=-3.241,Z=-3.434,all P<0.05);The total score of PCL-5 and the scores of four symptom groups in the PTSD group were higher than those in the control group(Z=-6.736,-6.924,-6.668,-6.856,-6.771,all P<0.05);Pearson correlation analysis showed that there was a significant negative correlation between the total score of PCL-5 and the 7 memory function scores of WMS-RC in the PTSD group(P<0.05).Intrusion symptoms were negatively correlated with all memory functions except visual regeneration(P<0.05),avoidance symptoms were negatively correlated with all memory functions except visual recognition,visual regeneration and tactile memory(P<0.05),and negative mood change,high alertness symptoms were significantly negatively correlated with all memory function scores(P<0.05).Conclusion:Patients with PTSD have impaired memory function,and the impairment of memory function becomes more obvious as the symptoms of PTSD increase.Among them,negative mood change and high alertness had significant effects on the impairment of memory function.
