摘 要:目的分析大动脉炎(Takayasu arteritis,TA)患者主动脉壁中有无淋巴细胞和浆细胞的表达,了解适应性免疫细胞是否参与TA的发病。方法对阜外医院行外科手术治疗的8例诊断为TA患者进行回顾性研究,研究其大动脉壁石蜡包埋组织切片免疫组织化学染色,计数CD20+B细胞,CD138+浆细胞以及CD4+T细胞的数量。结果8例TA患者男女比例1∶1,平均年龄(34.5±15.78)岁,病程平均为(46.13±37.47)个月。8例TA患者中,6例动脉壁中可见大量CD4+辅助T细胞和/或CD20+B细胞灶性浸润,3例见CD138+浆细胞灶性浸润,此3例患者红细胞沉降率均增高。结论TA大动脉壁内有辅助T细胞、B细胞和浆细胞浸润,提示适应性免疫反应,包括体液免疫反应参与了TA发病。Objective To analyze the infiltration of lymphocytes and plasma cells in the aortic wall of patients with Takayasu arteritis(TA)and explore the involvement of adaptive immune response in the pathogenesis of TA.Methods The clinical data of 8 TA patients who had large artery surgery in Fuwai Hospital was retrospectively studied.The paraffin embedded section of aortic wall was tested by immunohistochemistry and the number of CD20+B cells,CD138+plasma cells,CD4+T helper cells were counted.Results The males to female ratio was 1∶1,and the average age of these 8 patients was(34.5±15.78)years old.The average disease course was(46.13±37.47)months.CD4+T helper cells and/or CD20+B cells infiltrating foci presented in the aortic wall of 6 patients,and CD138+plasma cell foci aggregated in 3 patients who had high levels of ESR.Conclusion T helper cells,B cells,and plasma cells infiltrate in aortic wall of TA patients suggesting humoral immunity involved in the pathogenesis of TA.
