冷刀锥切术与宫颈环形电切术治疗宫颈鳞状上皮内瘤变的疗效比较

摘　　要：目的:观察冷刀锥切术(CKC)与宫颈环形电切术(LEEP)治疗宫颈鳞状上皮内瘤变(CIN)的疗效。方法:分析2015年9月—2018年9月我院95例行手术治疗CIN患者,根据手术方式分为CKC组(n=42)和LEEP组(n=53)。观察手术情况、术后残留及复发情况,比较两组患者术前、术后12个月时人乳头瘤病毒(HPV)和薄层液基细胞学(TCT)检测结果差异,记录并发症发生情况。结果:LEEP组手术时间(12.28±2.73)min、术中出血量(12.28±2.73)ml、创面愈合时间(35.26±7.11)d及术后并发症发生率(5.66%)均小于CKC组,差异有统计学意义。两组术后残留率(χ^2=0.076,P=0.782>0.05)及术后12个月复发率(χ^2=0.958,P=0.328>0.05)差异无统计学意义。两组术后12个月时高危型HPV感染情况转阴率(χ^2=2.107,P=0.147>0.05)和TCT异常率(χ^2=0.072,P=0.789>0.05)差异无统计学意义。结论:LEEP与CKC治疗CIN效果相当,LEEP在手术情况、术后并发症等方面较CKC更具优势。Objective To observe the efficacy of cold knife conization(CKC)and loop electrosurgical excision procedure(LEEP)in the treatment of cervical intraepithelial neoplasia(CIN).Methods The clinical data of 95 CIN patients undergoing surgical treatment were retrospectively analyzed in our hospital from September 2015 to September 2018.According to the surgical procedure,they were divided into CKC group(n=42)and LEEP group(n=53).The operation,postoperative residual and recurrence were observed.The detection results of human papillomavirus(HPV)and thinprep cytology test(TCT)were compared between the two groups before operation and at 12 months after operation,and the occurrence of complications was recorded.Results The operative time(12.28±2.73)min,intraoperative blood loss(12.28±2.73)ml,wound healing time(35.26±7.11)d and incidence rate of postoperative complications(5.66%)in LEEP groupwere lower than those in CKC groups.There were no significant differences in the residual rate after operation(χ^2=0.076,P=0.782﹥0.05)and recurrence rate at 12 months after operation(χ2=0.958,P=0.328﹥0.05)between the two groups.There were no significant differences in high-risk HPV infection(χ^2=2.107,P=0.147﹥0.05)and TCT abnormality rate(χ^2=0.072,P=0.789﹥0.05)between the two groups before operation and at 12 months after operation.Conclusions LEEP has equivalent effectswith CKC in the treatment of CIN.LEEP is superior to CKC in terms of operation and postoperative complications.
