肺低分化鳞状细胞癌罕见部位转移1例并文献复习

摘　　要：目的:探讨1例罕见肺低分化鳞状细胞癌转移病例的临床病理学特征、诊断、鉴别诊断及预后。方法:收集1例肺低分化鳞状细胞癌转移至前列腺的临床资料,对其进行HE及免疫组织化学染色,并复习相关文献。结果:前列腺MRI平扫增强提示前列腺恶性肿瘤合并转移;送检前列腺穿刺标本,肿瘤细胞粘附性较差,呈巢团浸润性生长,可见较多凝固性坏死;肿瘤细胞异型性大,可见较多病理性核分裂象及肿瘤巨细胞,周围未见正常前列腺腺泡结构;免疫表型:CK34βE12、CK5/6、P63、P40、CD31、D2-40阳性,PSA、P504S阴性,Ki-67增殖指数45%。患者采取治疗的方法为全身化疗和前列腺局部放疗。结论:肺低分化鳞状细胞癌转移至前列腺罕见,掌握临床病理特征对该病临床诊断、治疗及预后均有重要意义。Objective:To explore the clinicopathological characteristics,diagnosis,differential diagnosis and prognosis of a rare case of metastatic lung squamous cell carcinoma.Methods:The clinical data of one case of poorly differentiated squamous cell carcinoma of the lung metastasized to the prostate were collected,HE and immunohistochemical staining were performed,and the relevant literatures were reviewed.Results:Enhanced MRI scan of the prostate showed that prostate cancer with metastasis.Prostate puncture specimen was examined.Tumor cells had poor adhesion,grew in nests and more coagulative necrosis was visible.Tumor cells were heterogeneous,and multinucleated giant cells and pathological mitotic figures can be seen.No normal prostate tissue.The tumor cells were positive for CK34βE12,CK5/6,P63,P40,CD31 and D2-40,negative for PSA and P504 S,and that the Ki-67 labeling index was 45%.Patients are treated with systemic chemotherapy and local radiotherapy for the prostate.Conclusions:Low-differentiated squamous cell carcinoma of the lung metastasizes to the prostate is rare.Understanding the clinicopathological features is important for the clinical diagnosis,treatment and prognosis of the disease.
