腹腔镜经腹经肛管内外括约肌间切除术治疗超低位直肠癌的疗效

摘　　要：目的探讨超低位直肠癌患者予以腹腔镜经腹经肛管内外括约肌间切除术的效果。方法采用随机数字表法将我院收治的超低位直肠癌患者80例分为对照组和观察组,每组40例。对照组采用经腹直肠癌前切除术(Dixon术),观察组采用腹腔镜经腹经肛管内外括约肌间切除术,比较两组临床指标、肛门功能、并发症、复发情况及远处转移情况。结果两组术中出血量、肛门排气时间、住院时间、开始饮食时间、手术时间比较,P均<0.05;同组手术前后Wexner评分比较,P<0.05;两组术后Wexner评分、术后并发症发生率、术后1年复发率比较,P均<0.05;术后随访1年,两组远处转移率比较,P>0.05。结论超低位直肠癌患者予以腹腔镜经腹经肛管内外括约肌间切除术治疗有利于患者术后恢复,可有效改善患者肛门功能,且安全性较高。Objective To explore the effect of laparoscopic transabdominal transanal intersphincterectomy on ultralow rectal cancer.Methods 80 ultra-low rectal cancer patients admitted to our hospital were randomly divided in the con⁃trol group and the observation group,40 cases in each group.The control group underwent transabdominal rectal anterior resection(Dixon),and the observation group underwent laparoscopic transabdominal transanal intersphincterectomy.The clinical indicators,anal function,complications,recurrence and distant metastasis were compared between the two groups.Results The intraoperative blood loss,anal exhaust time,hospitalization time,time to start eating and operation time in the observation group were lower than those in the control group(P<0.05).After surgery,the Wexner scores of the two groups were lower than those before operation,and the observation group was lower than the control group;the incidence of postoperative complications in the observation group was lower than that in the control group;the recurrence rate of 1 year after operation in the observation group was lower than that of the control group(all P<0.05).After 1 year of followup,the distant metastasis rates were compared between the two groups with P>0.05.Conclusion The treatment of ultralow rectal cancer with laparoscopic transabdominal transanal intersphincterectomy is beneficial to the patient's postopera⁃tive recovery,which can effectively improve the patient's anal function and is safe.
