血清Flotillin表达对阿尔茨海默病的早期诊断价值

摘　　要：目的检测阿尔茨海默病(AD)早期患者血清中浮舰蛋白(Flotillin)水平,同时进行精神状态量表(MMSE)评分,并探讨Flotillin mRNA水平对AD的早期诊断价值。方法选取52例AD早期患者为研究对象(AD组),同期选取68例健康体检者作为对照(对照组)。采用实时荧光定量PCR(qRT-PCR)法检测两组受试者血清中Flotillin mRNA水平,并进行MMSE评分;分析AD患者血清Flotillin mRNA水平与MMSE总评分相关性;分析血清Flotillin mRNA水平对AD的早期诊断价值;Logistic回归分析影响AD发生的危险因素。结果AD组血清Flotillin mRNA水平高于对照组,MMSE总评分、定向力评分、记忆力评分、计算力评分、回忆评分、语言评分均低于对照组,差异有统计学意义(P<0.05);AD患者血清中Flotillin mRNA水平与MMSE总评分呈负相关(r=-0.586,P=0.000);血清Flotillin mRNA诊断AD的曲线下面积(AUC)为0.842,截断值为1.486,特异性为88.2%,敏感度为69.2%;Flotillin mRNA高表达、MMSE低评分是影响AD发生的危险因素。结论血清Flotillin mRNA水平在AD患者中明显升高,且与MMSE总评分呈负相关。血清Flotillin mRNA可能有助于疾病早期诊断,对评价AD的早期危险性有重要意义。Objective To detect the level of serum Flotillin in patients with early Alzheimer's disease(AD),evaluate the Mini-Mental State Examination(MMSE)score,and explore the value of Flotillin mRNA in the diagnosis of early AD.Methods From August 2018 to December 2019,52 patients with AD were selected as AD group,and 68 healthy people were selected as control group.Real-time fluorescent quantitative PCR(qRT-PCR)was used to detect the expression of Flotillin mRNA in the serum,and MMSE score was performed;the correlation between serum Flotillin mRNA level and MMSE total score in AD patients was analyzed;and the value of serum Flotillin mRNA level in the diagnosis of early AD was analyzed;the risk factors of AD were analyzed by Logistic regression analysis.Results The level of serum Flotillin mRNA in AD group was higher than that in the control group,and the MMSE total score,orientation score,memory score,calculation score,palinmnesis score and language score were lower than those in the control group,the differences were statistically significant(P<0.05);there was a negative correlation between the level of serum Flotillin mRNA and MMSE total score(r=-0.586,P=0.000);the area under the curve(AUC)of serum Flotillin mRNA in the diagnosis of AD was 0.842,the cutoff value was 1.486,the specificity was 88.2%,and the sensitivity was 69.2%;Flotillin mRNA high expression and low MMSE score were risk factors affecting AD.Conclusions The level of serum Flotillin mRNA is significantly increased in AD patients,and is negatively correlated with MMSE total score.Serum Flotillin mRNA might be helpful for the diagnosis of early AD and the evaluation of the risk factors for early AD.
