摘　　要：目的研究计算机断层扫描(CT)引导下碘(125I)粒子植入术治疗肺癌骨转移的效果与安全性。方法回顾性分析2016年7月至2019年7月在该院接受CT引导下125I粒子植入术的60例肺癌骨转移性患者病例资料。比较术前与术后患者的疼痛数字评分(NRS)情况、肿瘤最大径变化。参照RECIST标准(Version 1.1),分为完全缓解(CR)、部分缓解(PR),疾病稳定(SD)和疾病进展(PD),评估患者治疗效果。观察患者并发症发生情况,随访期为2~6个月。结果治疗后32例疼痛CR,17例PR,止痛有效率81.6%。治疗后26例局部病灶CR,25例PR,15例SD,4例PD,肿瘤缓解率68.3%。所有患者均无严重并发症发生。结论对于肺癌骨转移患者,采用CT导向引导下125I粒子植入可减轻疼痛,缩小肿瘤体积,有效控制肿瘤局部进展,缓解患者病痛,建议临床推广应用。Objective To study the effect and safety of iodine(I)-125 seed implantation under the guidance of computed tomography(CT)in the treatment of lung cancer bone metastases.Methods The case data of 60 patients with lung cancer and bone metastases who underwent CT-guided 125I seed implantation in the hospital from July 2016 to July 2019 were retrospectively analyzed.The patients′pain numerical score(NRS)before and after surgery and the change of the maximum tumor diameter were compared.According to the RECIST standard(Version 1.1),it was divided into complete remission(CR),partial remission(PR),stable disease(SD)and disease progression(PD)to evaluate the treatment effect of patients.The patient′s complications were observed,and the follow-up period was 2-6 months.Results After treatment,32 cases were pain CR,17 cases were PR,the effective rate of analgesia was 81.6%.After treatment,26 cases of local lesions were CR,25 cases were PR,15 cases were SD,4 cases were PD,and the tumor remission rate was 68.3%.All patients had no serious complications.Conclusion For patients with lung cancer bone metastases,CT-guided 125I seed implantation can reduce pain,reduce tumor volume,effectively control local tumor progression,and relieve patient pain.It is recommended for clinical application.
