摘　　要：目的研究经会阴前列腺穿刺术患者发生迷走反射的风险因素。方法回顾性纳入海军军医大学附属长海医院泌尿中心手术室局部麻醉下经会阴前列腺穿刺患者729例,并记录患者体种指数(BMI)、年龄、既往史、手术史、穿刺针数、术晨血压及疼痛评分。采用Logistic回归模型分析相关风险因素,P<0.05为差异有统计学意义。结果729例患者的平均年龄(67.78±8.581)岁,发生迷走反射62例(8.5%)。回归分析发现患者围术期疼痛(OR=79.832,P<0.001)、术晨血压异常(OR=4.415,P<0.001)、穿刺针数(OR=3.123,P=0.005)及体重指数(OR=2.255,P=0.044)均具有统计学意义。结论围手术期的疼痛、术晨血压不稳定、穿刺针数以及BMI均是诱发术后迷走反射的风险因素。Objective To explore the risk factors of vasovagal reactions after transperineal prostate biopsy.Methods Clinical data of 729 prostate cancer(PCa)patients who underwent transperineal prostate biopsy under local anesthesia in our hospital were retrospectively analyzed,including body mass index(BMI),age,past history,surgical history,number of punctures,morning blood pressure on the operation day and pain score.The risk factors were analyzed with Logistic regression model.Results The patients average age was(67.78±8.581)years.Vasovagal reactions occurred in 62 cases(8.5%).Regression analysis revealed that perioperative pain(OR=79.832,P<0.001),abnormal morning blood pressure(OR=4.415,P<0.001),number of punctures(OR=3.123,P=0.005)and BMI(OR=2.255,P=0.044)were risk factor of vasovagal reactions after transperineal prostate biopsy under local anesthesia.Conclusion Perioperative pain,abnormal morning blood pressure,number of punctures and BMI are risk factors inducing vasovagal reactions after transperineal prostate biopsy.
