结肠癌造口患者健康行为影响因素分析

摘　　要：目的结肠癌为临床致死率较高的恶性肿瘤,外科手术可有效提升患者生存率,但术后仍有部分患者健康行为受到影响。本研究分析结肠癌造口患者健康行为现状影响因素,为改善结肠癌造口患者健康行为提供依据。方法选取2014-12-05-2019-12-10驻马店市中心医院收治的91例结肠癌造口患者作为研究对象,采用健康行为促进生活方式量表Ⅱ对所有患者进行健康行为调查,根据调查结果,将评分>171分设为健康行为良好组(n=41),评分≤171分设为健康行为一般组(n=50)。采用单因素与多因素分析肠癌造口患者健康行为一般的影响因素。结果单因素分析显示,文化程度(χ2=7.995,P=0.018)、家庭月收入(χ2=10.003,P=0.002)、化疗损伤(χ2=11.230,P=0.001)和造口并发症(χ2=8.444,P=0.004)与结肠癌造口患者健康行为有关联。多因素Logistic回归分析结果显示,文化程度(OR=4.09,95%CI为1.574~15.314,P=0.006)、家庭月收入(OR=4.018,95%CI为1.666~9.689,P=0.002)、化疗损伤(OR=4.471,95%CI为1.820~10.979,P=0.001)及造口并发症(OR=3.810,95%CI为1.511~9.605,P=0.005)是结肠癌造口患者健康行为的影响因素。结论初中及以下文化程度、家庭月收入<3000元、化疗损伤、造口并发症的结肠癌造口患者健康行为相对较差,临床需重点关注并予以干预,帮助患者建立健康的生活方式,有利于提升结肠癌造口患者健康行为水平。OBJECTIVE Colon cancer is a malignant tumor with a high clinical mortality rate.Surgery can effectively improve the survival rate of patients,but the health behavior of some patients is still affected after surgery.This study analyzed the factors affecting the health behavior of patients with colon cancer stoma and provided a basis for improving the health behavior of patients with colon cancer stoma.METHODS A total of 91patients with colon cancer stoma admitted to Zhumadian Central Hospital from December 5,2014to December 10,2019were selected as the research subjects.The Healthy Behavior Promotional Lifestyle ScaleⅡwas used to conduct a healthy behavior survey on all patients.According to the survey results,Set the score>171points as the healthy behavior group(n=41),and the score≤171 points as the normal healthy behavior group(n=50).Single-factor and multi-factor analysis were used to find out the general influencing factors of health behavior of patients with colon cancer stoma.RESULTS Univariate analysis showed that educational level(χ2=7.995,P=0.018),family monthly income(χ2=10.003,P=0.002),chemotherapy injury(χ2=11.230,P=0.001)and stoma complications(χ2=8.444,P=0.004)was related to the health behavior of patients with colon cancer stoma.Multivariate logistic regression analysis showed that education level(OR=4.09,95%CI:1.574-15.314,P=0.006),family monthly income(OR=4.018,95%CI:1.666-9.689,P=0.002),chemotherapy injury(OR=4.471,95%CI:1.820-10.979,P=0.001)and stoma complications(OR=3.810,95%CI:1.511-9.605,P=0.005)were the influences on the health behavior of patients with colon cancer stoma factor.CONCLUSIONS Colon cancer stoma patients with junior high school education and below,family monthly income<3000yuan,chemotherapy injury and stoma complications are relatively have poor health behavior,clinical attention and intervention are needed to help patients establish a healthy lifestyle.It is beneficial to improve the health behavior level of patients with colon cancer stoma.
