摘  要:目的分析血清β-葡萄糖苷酶(CBG)、淀粉样蛋白A (SAA)、热休克蛋白70 (HSP-70)联合检测在新生儿坏死性小肠结肠炎(NEC)早期诊断中的应用价值。方法选取2016年3月-2019年2月在台州市立医院就诊的47例NEC患儿为观察组,同期47例健康新生儿为对照组。采用ELISA法检测血清CBG、SAA、HSP-70水平,分析观察组患儿血清CBG、SAA、HSP-70表达的相关性。结果观察组血清CBG、SAA、HSP-70水平均显著高于对照组(均P <0.05)。Ⅰ期、Ⅱ期、Ⅲ期观察组患儿血清CBG、SAA、HSP-70水平比较差异均有统计学意义(均P<0.05)。Ⅲ期患儿血清CBG、SAA、HSP-70水平显著高于Ⅱ期患儿,Ⅱ期患儿血清CBG、SAA、HSP-70水平显著高于Ⅰ期患儿(均P<0.05)。观察组血清CBG与SAA、HSP-70表达均呈正相关(均P<0.05),血清SAA与HSP-70表达呈正相关(P<0.05)。三者联合检测诊断NEC的ROC曲线下面积为0.969,灵敏度为89.47%,特异度为90.93%。结论 NEC患儿血清CBG、SAA、HSP-70水平均上调,三者均参与NEC的发生、发展,可作为诊断早期NEC的指标。Objective To analyze the application value of joint detection of serum β-glucosaccharase (CBG),amyloid protein A (SAA),and heat shock protein 70 (HSP-70) in early diagnosis of neonatal necrotizing enterocolitis (NEC). Methods From March2016 to February 2019,47 neonates with NEC were selected from Taizhou Municipal Hospital as observation group,and 47 healthy neonates were selected as control group. ELISA was used to detect the levels of serum CBG,SAA,and HSP-70,the correlations of serum CBG,SAA,and HSP-70 expressions in observation group were analyzed. Results The levels of serum CBG,SAA,and HSP-70 in observation group were statistically significantly higher than those in control group (P<0. 05). There were statistically significant differences in the levels of serum CBG,SAA,and HSP-70 among the neonates of stage Ⅰ,stage Ⅱ,and stage Ⅲ in observation group (P<0. 05). The levels of serum CBG,SAA,and HSP-70 in neonates of stage Ⅲ were statistically significantly higher than those in neonates of stage Ⅱ,the levels of serum CBG,SAA,and HSP-70 in neonates of stage Ⅱ were statistically significantly higher than those in neonates of stage Ⅰ (P<0. 05). In observation group,serum CBG expression was positively correlated with SAA expression and HSP-70 expression (P<0. 05),serum SAA expression was positively correlated with HSP-70 expression (P<0. 05). The area under ROC curve,sensitivity,and specificity of joint detection of serum CBG, SAA, and HSP-70 in diagnosis of NEC were 0. 969, 89. 47%, and 90. 93%, respectively.Conclusion The levels of serum CBG,SAA,and HSP-70 in NEC neonates increase,which are involved in occurrence and development of NEC,the three markers can be used to diagnose early NEC.
