摘 要:目的探讨血清总前列腺特异抗原(tPSA)、前列腺特异抗原密度(PSAD)和血清游离前列腺特异抗原百分率(FPSAR)水平对前列腺癌病理分期的预测价值。方法选取2016年10月至2019年10月间汉中市中心医院收治的60例前列腺癌患者,分析不同病理分期患者的血清t-PSA、PASD和FPSAR水平差异,采用工作特征曲线(ROC)比较以上指标对前列腺癌病理分期的预测价值。结果T3、T4期血清t-PSA水平均高于T2期水平,且T4期高于T3期,差异均有统计学意义(P<0.05)。T4期血清PSAD水平高于T2、T3期,差异均有统计学意义(P<0.05)。各分期患者FPSAR水平比较,差异无统计学意义(P>0.05)。血清t-PSA、PASD和FPSAR预测前列腺癌病理分期的AUC分别为0.896、0.880和0.873,均有一定预测价值。最佳临界值预测敏感度和特异度:t-PSA为92.6%和81.8%,PASD为81.5%和87.9%,FPSAR为100.00%和57.6%。FPSAR对前列腺癌病理分期的预测敏感度最高,但就AUC而言,血清t-PSA是三项指标中前列腺癌病理分期预测最好的一项。结论血清t-PSA、PASD和FPSAR三项指标单独预测前列腺期病理分期的效能均较高,对前列腺癌病理分期诊断有较好的辅助作用,值得在临床诊断中推广。Objective To investigate the predictive value of serum levels of total prostate-specific antigen(t-PSA),prostate-specific antigen density(PSAD)and serum free prostate-specific antigen ratio(FPSAR)for pathological staging of prostate cancer.Methods Sixty patients with prostate cancer admitted to Hanzhong Central Hospital from October 2016 to October 2019 were selected as the research subjects.Difference in serum t-PSA,PASD and FPSAR level o was analyzed in patients of different pathological staging of prostate cancer.Receiver operating characteristic(ROC)curve was used to compare the predictive value of the above indexes for the pathological staging of prostate cancer.Results t-PSA level was higher in patients with T3 or T4 prostate cancer than in patients with T2 prostate cancer and higher in patients with T4 prostate cancer than in patients with T3 prostate cancer(all P<0.05).PSAD level was higher in patients T4 prostate cancer than in patients with T2 and T3 prostate cancer(all P<0.05).There was no significant difference in the level of FPSAR among patients of different pathological staging of prostate cancer(P>0.05).Area under the curve(AUC)of serum t-PSA,PASD and FPSAR was 0.896,0.880,0.873,respectively for the prediction of pathological staging of prostate cancer.The sensitivity and specificity for the best critical value was 92.6%and 81.8%,respectively for t-PSA,81.5%and 87.9%,respectively for PSAD and 100.00%and 57.6%,respectively for FPSAR.FPSAR had the highest sensitivity for pathological staging of prostate cancer and t-PSA showed the best predictive efficacy based on AUC.Conclusion The three indexes of serum t-PSA,PASD and FPSAR can effectively predict the pathological staging of prostate alone,which can be complementally used for clinical diagnosis of prostate cancer.
