摘　　要：目的探讨糖类抗原19-9(CA19-9)水平预测纳米刀消融术治疗局部进展期胰腺癌(LAPC)患者预后的价值。方法根据纳米刀消融术后第30天CA19-9水平与术前差值分为升高组(16例)和下降组(39例),检测两组术前和术后第1、7、30天CA19-9水平。绘制两组患者生存曲线并比较生存时间,分析术后第30天CA19-9差值与生存时间的相关性。结果升高组术后第1、7、30天CA19-9水平均高于术前(P<0.05),术后第30天仍高于术前[(397.72±153.61)U/mL vs.(231.43±98.43)U/mL](P<0.05)。下降组术后第30天CA19-9水平低于术前[(154.63±130.92)U/mL vs.(406.01±290.67)U/mL](P<0.05),且低于升高组(P<0.05)。升高组中位生存时间短于下降组(6个月vs.16个月)(P<0.01)。术后第30天CA19-9差值与患者生存时间呈正相关(r=0.320,P<0.05)。结论CA19-9水平可作为预测纳米刀消融术治疗LAPC患者预后有价值的指标。Objective To investigate the value of carbohydrate antigen 19-9(CA19-9)level in predicting the prognosis of patients underwent irreversible electroporation ablation(IREA)for locally advanced pancreatic cancer(LAPC).Methods According to difference of CA19-9 level on the 30th day after operation,the patients underwent IREA for LAPC were assigned into two groups of A(with increased CA19-9 level,16 cases)and B(with decreased CA19-9 level,39 cases).Serum CA19-9 levels were detected before and on the 1st,7th and 30th day after IREA were detected.The survival curves of two groups were drawn to compare the survival times.The correlation between the CA19-9 levels on the 30th day after operation and the survival time was analyzed.Results Serum CA19-9 levels on the 1st,7th and 30th day after operation were higher than those before in group A(P<0.05),which on the 30th day after operation was still higher than that before[(397.72±153.61)U/mL vs.(231.43±98.43)U/mL](P<0.05).Serum CA19-9 level on the 30th day after operation in group B was lower than that before[(154.63±130.92)U/mL vs.(406.01±290.67)U/mL](P<0.05)and that in group A(P<0.05).The median survival time was shorter in group A than that in group B(6 months vs.16 months)(P<0.01).Serum CA19-9 level on the 30th day after operation was positively correlated with the survival time(r=0.320,P<0.05).Conclusion Serum CA19-9 level can be used as a valuable indicator in predicting the prognosis of the patients underwent IREA for LAPC.
