摘 要:目的比较乳腺良、恶性疾病MRI检查背景实质强化(BPE)分级、动态增强半定量指标及表观扩散系数(ADC)。方法回顾性分析乳腺MRI检查并经病理证实的乳腺疾病患者75例资料。其中,良性病变57例(良性组),恶性病变18例(恶性组)。比较两组患者BPE分级、ADC以及动态增强半定量指标:达峰时间(TTP),正性增强积分(PEI),时间最大密度投影(TMIP),最大上升斜率(MSI),流入(WI)和流出(WO)。结果两组患者病灶BPE时间-信号曲线类型均为Ⅰ型,BPE分级差异有统计学意义(P<0.05)。良性组内纤维腺瘤、腺病、乳腺腺病伴纤维腺瘤、导管内乳头状瘤间的BPE分级差异无统计学意义(P>0.05)。恶性组内浸润性导管癌、导管内乳头状癌、黏液腺癌间的BPE分级差异亦无统计学意义(P>0.05)。良性组患者ADC和TTP高于恶性组,MSI低于恶性组(P<0.05),而两组间PEI、TMIP、WI、WO比较差异无统计学意义(P>0.05)。结论乳腺良、恶性病灶MRI检查BPE分级、TTP、MSI与ADC存在差异。各型乳腺疾病BPE时间-信号曲线类型均为Ⅰ型,不能用于鉴别良、恶性乳腺病灶。Objective To compare the background parenchymal enhancement(BPE)classification,dynamic enhanced semi-quantitative index and apparent diffusion coefficient(ADC)of breast benign and malignant diseases on MRI.Methods The clinical data of 75 patients with breast diseases underwent breast MRI and pathological examination were analyzed retrospectively,of whom benign lesions were diagnosed in 57 cases(group A)and malignant lesions were diagnosed in 18 cases(group B).The BPE classification,ADC and dynamic enhanced semi-quantitative parameters including the time to peak(TTP),positive enhancement integral(PEI),time maximum intensity projection,maximum ascending slope(MSI),inflow(WI)and outflow(WO)were compared between the two groups.Results The BPE time-signal curve type of the lesions in two groups were typeⅠin all with significant difference in BPE classification(P<0.05).There was no significant difference in BPE classification among benign intraductal fibroadenoma,adenopathy,breast adenopathy with fibroadenoma and intraductal papilloma(P>0.05).There was no significant difference in BPE classification among invasive ductal carcinoma,intraductal papillary carcinoma and mucinous adenocarcinoma(P>0.05).ADC and TTP were higher and MSI was lower in group A than those in group B(P<0.05),but there were no significant differences in PEI,TMIP,WI and WO between the two groups(P>0.05).Conclusion There are differences in BPE classification,TTP,MSI and ADC of benign and malignant breast lesions on MRI.The type of BPE time-signal curve of various types of breast diseases is typeⅠ,which can not be used to distinguish the benign lesions from the malignant.
