摘　　要：目的探究趋化因子受体7(CCR7)和D2-40标记的微淋巴管密度(MLVD)在甲状腺癌组织的表达及其与淋巴结转移的相关性。方法收集武汉市中心医院2019年1月至2019年10月期间67例确诊为甲状腺癌的患者病理标本制作石蜡切片,选择同期67例正常甲状腺组织标本作为对照组,均通过免疫组织化学方法进行CCR7和D2-40检查。观察两组患者CCR7和D2-40的表达量,分析CCR7、D2-40与淋巴结转移的相关性。结果甲状腺癌组CCR7阳性表达率及D2-40标记的MLVD水平显著高于正常对照组(P<0.05);淋巴转移组阳性表达率及D2-40标记的MLVD水平显著高于无淋巴转移组(P<0.05);病理类型、淋巴是否转移等组间CCR7、D2-40标记的MLVD检测结果比较,差异均有统计学意义(P<0.05);Spearman相关分析显示,D2-40标记的MLVD与CCR7表达呈正相关(相关系数rs=0.643,P<0.000),D2-40标记的MLVD与各项目关系应用直线相关分析,D2-40标记的MLVD与病理类型、淋巴结是否转移明显相关(P<0.05),与年龄、T分期无明显相关(P>0.05)。结论CCR7和D2-40标记的MLVD高表达与淋巴转移密切相关。Objective To investigate the expression of chemokine receptor-7(CCR7)and D2-40 labeled lymphatic microvessel density(MLVD)in thyroid carcinoma and its correlation with lymph node metastasis.Methods The thyroid tissues of 67 patients with thyroid cancer in wuhan central hospital from January to September in 2019 were selected as the thyroid group,and 67 normal thyroid tissues of healthy people were selected as the healthy control group.CCR7 and D2-40 were examined by immunohistochemistry.The results of CCR7 and D2-40 were observed,and the correlation between CCR7,D2-40 and lymph node metastasis was analyzed.Results The positive expression rate of CCR7 and the level of MLVD labeled with D2-40 in thyroid cancer group were significantly higher than those in healthy control group(P<0.05);the positive expression rate of CCR7 and the level of MLVD labeled with D2-40 in lymph node metastasis group were significantly higher than those in non lymph node metastasis group(P<0.05);There were significant differences in the detection results of CCR7 and D2-40 labeled MLVD among pathological types and lymph node metastasis groups(P<0.05);Spearman correlation analysis showed that the expression of CCR7 was positively correlated with D2-40 labeled MLVD(correlation coefficient rs=0.643,P<0.01),and the relationship between D2-40 labeled MLVD and each item was analyzed by linear correlation analysis MLVD was correlated with pathological type and lymph node metastasis(P<0.05),but not with age and T stage(P>0.05).Conclusion The high expression of MLVD labeled with CCR7 and D2-40 is closely related to lymph node metastasis,which suggests that it is necessary to strengthen the detection of the above indicators in patients with thyroid cancer.At the same time,in order to improve the accuracy of detection results,the above indicators can be combined in the evaluation of patients with thyroid cancer,so as to judge whether the patients have lymph node metastasis,so as to facilitate the clinical targeted treatment to prevent or delay The dise
