摘  要:目的分析研究鼻黏膜息肉的3.0T1H-MRS波谱图像特点,探讨3.0T1H-MRS对鼻黏膜息肉的诊断价值。方法随机从我科选取2010年1月~2019年6月收治的鼻黏膜息肉患者50例进行研究,患者均得到病理验证,均实施3.0T MRI、3.0T1H-MRS病灶检测,并和同侧额部正常脑组织进行对比量化分析,主要研究分析乙酸(Ace)、乳酸(Lac)、N-乙酰天门冬氨酸(NAA)、胆碱(Cho)、肌酸(Cr)各峰积分数值并进行比较:Cho/NAA、Cr/Lac、Cr/NAA、Cho/Cr、Lac/NAA和Lac/Ace。结果 3.0T1H-MRS在鼻息肉中的表达与正常脑组织有显著差异:NAA(17.76±6.20)降低,Cho(23.19±6.91)升高,Lac(12.51±3.42)升高,Ace(11.43±6.11)升高,最为明显;Lac/NAA(0.70±0.55)比值升高,Cho/NAA(1.34±1.10)比值升高,且更为明显;Lac/Ace(1.09±0.56)比值下降且最为明显;两组比较差异有统计学意义(P<0.05)。结论在其他影像学影像检查的基础上,对鼻窦黏膜息肉进行3.0T1H-MRS波谱检测,能对鼻黏膜息肉做出正确诊断。Objective To analyze and study the 3.0T1H-MRS characteristics of nasal polyps,and to explore the value of 3.0T1H-MRS in the diagnosis of nasal polyps.Methods 50 patients with nasal polyps admitted to our department from January 2010 to June 2019 were randomly selected for the study.All the patients were confirmed by pathology,and 3.0T was implemented MRI,3.0t1h-mrs were used to detect the lesions,and quantitative analysis was made with the ipsilateral normal brain tissue.The peak integral values of acetic acid(ACE),lactic acid(LAC),N-acetylaspartate(NAA),choline(CHO),creatine(CR)were analyzed and compared:Cho/NAA,Cr/lac,Cr/NAA,CHO/Cr,lac/NAA and lac/ace.Results The expression of 3.0T1H-MRS in nasal polyps was significantly different from that in normal brain tissues:NAA(17.76±6.20)decreased,Cho(23.19±6.91)increased,Lac(12.51±3.42)increased,Ace(11.43±6.11)increased,Lac/NAA(0.70±0.55)ratio increased,Cho/NAA ratio(1.34±1.10)increased,and more obvious;Lac/Ace(1.09±0.56)ratio decreased and the most obvious The difference between the two groups was statistically significant(P<0.05).Conclusions On the basis of other imaging examinations,3.0T1H-MRS spectrum detection can make a correct diagnosis of nasal mucosal polyps.
