摘　　要：目的探讨3.0T磁共振成像(MRI)表现对脑脂肪栓塞(CFE)的诊断价值。方法回顾分析6例临床确诊为脂肪栓塞综合征(FES)患者的头颅MRI影像学资料。结果MRI显示病灶散在、多发分布于双侧基底节区、半卵圆中心、放射冠区、丘脑、脑室旁深部脑白质及水岭区脑皮质区,两侧分布基本对称,病灶边界不清,边缘模糊。弥散加权成像(DWI)均出现多发高亮"星空样"病灶;磁敏感加权成像(SWI)脑内见弥漫性细密低信号影。发病早期病灶MRI显示敏感性SWI>DWI>flair>T2>T1。随DWI脑内病灶显示减少,患者临床症状不同程度减轻、好转;SWI细密低信号灶持续存在。结论MRI对CFE的影像学诊断具有一定的特征性,可作为早期诊断CFE的常规方法。Objective To explore the diagnostic value of 3.0T magnetic resonance imaging(MRI)for cerebral fat embolism(CFE).Methods The head MRI imaging data of 6 patients with fat embolism syndrome(FES)were retrospectively analyzed.Results MRI showed that the lesions were scattered and frequently distributed in the bilateral basal ganglia,semi-oval center,radial coronal area,thalamus,paraventricular deep white matter,and watershed cerebral cortex.The distribution on both sides was basically symmetrical,and the borders of the lesions were unclear and the edges blurry.Diffusion-weighted imaging(DWI)showed multiple bright"star-like"lesions;susceptibility weighted imaging(SWI)showed diff use and fi ne low-intensity shadows in the brain.MRI of the early lesions showed sensitivity SWI>DWI>fl air>T2>T1.With the reduction of DWI brain lesions,the clinical symptoms of patients were reduced and improved to varying degrees;SWI fi ne low-intensity lesions persisted.Conclusion MRI has certain characteristics for the imaging diagnosis of CFE and can be used as a routine method for early diagnosis of CFE.
