摘 要:目的:探究机械性损伤输尿管建立狭窄模型的可行性。方法:将成年雄家兔通过手术机械损伤,建立左侧输尿管狭窄模型。术前、术后7、21、49 d采用静脉肾盂造影及核素肾动态显像,评估肾、输尿管形态与功能;病理学观察损伤段输尿管、同侧肾及对侧输尿管、肾改变;免疫组织化学、RT-PCR、免疫印迹观察输尿管愈合区α-平滑肌肌动蛋白(α-SMA)、Ⅰ型、Ⅲ型胶原及转化生长因子-β1(TGF-β1)表达。结果:静脉肾盂造影显示术后7 d模型兔左侧输尿管通畅,随着时间的延长左侧肾盂积水、输尿管扩张逐渐加重;术后7、21、49 d与术前相比,左肾肾小球滤过率及分肾摄取率逐步降低,达峰时间及半排时间逐渐延长;左侧肾小球萎缩,肾小管扩张,输尿管愈合区域成纤维细胞增殖和胶原纤维沉积,α-SMA在输尿管愈合区域表达,且TGF-β1、Ⅰ型、Ⅲ型胶原表达增加。结论:机械性损伤输尿管后狭窄的家兔模型,能够模拟输尿管狭窄的动态全过程,能够为输尿管狭窄研究提供良好模型。Objective:To explore the feasibility of the ureteral stricture formation model caused by mechanical injury.Methods:Adult male rabbits underwent a surgical procedure in a left ureteral stricture model.The morphology and function of the kidney and ureter were evaluated by intravenous pyelography and radionuclide renal scintigraphy during the 7 days,21 days,and 49 days after and before surgery,respectively.Pathology was responsible for the observed differences of kidney and contralateral ureter in the injured side.Using immunohistochemistry,RT-PCR and Western blotting we detected the expression of alpha-smooth muscle actin,Ⅰ,Ⅲcollagen and transforming growth factor-β1.Results:Intravenous pyelography showed the left ureter was unobstructed at the end of the 7th day after operation,and the hydronephrosis and ureteral dilatation on the left side increased gradually over time.The glomerular filtration rate and uptake rate of the left kidney decreased gradually.Meanwhile,the peak time and half drain time were gradually prolonged on the 7 d,21 d and 49 d after operation compared with those before operation.Left renal tubular dilatation and glomerular atrophy,fibroblast proliferation and collagen fibers deposition and expression of alpha-smooth muscle actin were observed in the ureteral healing area.These results suggested a profound increased expression of transforming growth factor-β1,ⅠandⅢcollagen.Conclusion:The rabbit model of mechanically injured ureteral stenosis can simulate the whole dynamic process of ureteral stenosis and provide a excellent model for ureteral stricture research.
