摘　　要：目的探讨甲状腺激素检测在精神分裂症患者中的应用价值。方法选取30例精神分裂症患者作为病例组,病例组按照阳性与阴性症状量表(PANSS)分为阳性亚组(n=18)和阴性亚组(n=12);选取同期健康体检者30例作为对照组。比较病例组与对照组,阳性亚组与阴性亚组的甲状腺激素水平。分析病例组甲状腺激素水平与精神分裂症的相关性。结果病例组血清FT3、FT4水平显著高于对照组(P<0.05),TSH水平显著低于对照组(P<0.05);阳性亚组血清FT3、FT4水平显著高于阴性亚组(P<0.05),TSH水平显著低于阴性亚组(P<0.05)。PANSS量表总得分与血清甲状腺激素(FT3、FT4、TSH)水平无相关性(P>0.05);PANSS量表一般精神病理评分与血清FT3、FT4水平呈正相关(P<0.05),与TSH水平呈负相关(P<0.05);PANSS量表阴性症状评分与TSH水平呈负相关(P<0.05),与FT3、FT4水平无相关性(P>0.05);PANSS量表阳性症状评分与FT3水平呈正相关(P<0.05),与TSH水平呈负相关(P<0.05),与FT4水平无相关性(P>0.05)。结论精神分裂症患者普遍存在血清甲状腺激素水平异常情况,对FT3、FT4、TSH水平进行动态观察可为病情及预后评估提供参考。Objective To explore the application value of thyroid hormone detection in patients with schizophrenia.Methods 30 patients with schizophrenia were selected as the case group,and divided into the the positive subgroup(n=18)and the negative subgroup(n=12)according to the positive and negative symptom scale(PANSS).30 healthy physical examinees at the same period were selected as the control group.The thyroid hormone levels of positive subgroup vs.negative subgroup,and case group vs.control group were compared,and the correlations between the thyroid hormone levels and schizophrenia in the case group were analyzed.Results The serum FT3 and FT4 levels in the case group were significantly higher than those in the control group(P<0.05),and TSH level was significantly lower than that in the control group(P<0.05).The serum FT3 and FT4 levels in the positive subgroup were significantly higher than those in the negative subgroup(P<0.05),and TSH level was significantly lower than that in the negative subgroup(P<0.05).The total score of PANSS scale had no correlation with serum thyroid hormone(FT3,FT4,TSH)levels(P>0.05);The general psychopathological score of PANSS scale was positively correlated with serum FT3 and FT4 levels(P<0.05),but negatively correlated with TSH level(P<0.05);The negative symptom score of PANSS scale was negatively correlated with TSH level(P<0.05),but had no correlation with FT3 and FT4 levels(P>0.05);The positive symptom score of PANSS scale was positively correlated with FT3 level(P<0.05),negatively correlated with TSH level(P<0.05),and had no correlation with FT4 level(P>0.05).Conclusions Abnormal serum thyroid hormone levels are common in patients with schizophrenia.Dynamic observation of FT3,FT4 and TSH levels can provide reference for the assessment of disease condition and prognosis.
