80例复治肺结核患者对一线抗结核药物的耐药性分析

摘　　要：目的:探讨复治肺结核患者对一线抗结核药物的耐药性。方法:抽取2019年1月至2020年5月郑州市第六人民医院收治的复治肺结核患者80例,对其痰标本进行实验室结核分枝杆菌培养及分离,分析菌株对一线抗结核药物的耐药情况。结果:80例复治肺结核患者痰标本共分离出结核分枝杆菌80株,其中耐一线抗结核药物45株(56.25%,45/80),包括耐1种一线抗结核药物药18株,耐2种一线抗结核药物药10株,耐3种一线抗结核药物8株,对一线抗结核药物均有耐药性9株。任一耐药中对链霉素(33.75%)的耐药性较高,其次为利福平(32.50%)。结论:复治肺结核患者对一线抗结核药物普遍存在不同程度耐药性,临床应加强复治肺结核患者对抗结核药物的耐药筛查,动态监测耐药情况,以指导抗结核药物的合理选择。Objective To investigate the resistance of first-line anti-tuberculosis drugs in 80 patients with retreated pulmonary tuberculosis.Methods Eighty patients with retreated pulmonary tuberculosis admitted to Zhengzhou Sixth People’s Hospital from January 2019 to May 2020 were selected.Their sputum samples were cultured and isolated from mycobacterium tuberculosis in the laboratory,and the resistance of the strains to first-line anti-tuberculosis drugs was analyzed.Results A total of 80 strains of Mycobacterium tuberculosis were isolated from the sputum samples of 80 retreated pulmonary tuberculosis patients.Among them,45 strains(56.25%,45/80)were resistant to first-line antituberculosis drugs,including 18 strains resistant to one first-line anti-tuberculosis drug,10 strains resistant to 2 first-line anti-tuberculosis drugs,8 strains resistant to 3 first-line anti-tuberculosis drugs,and 9 strains resistant to first-line anti-tuberculosis drugs.Streptomycin resistance was higher in any drug resistance(33.75%),followed by rifampicin(32.50%).Conclusions Patients with re-treated pulmonary tuberculosis generally have different degrees of resistance to first-line anti-tuberculosis drugs.Clinically,the drug resistance screening of retreated pulmonary tuberculosis patients should be strengthened,and dynamically monitor drug resistance to guide the rational choice of anti-tuberculosis drugs.
