摘  要:脑血管病是发生血管性认知障碍(VCI)的主要原因,MRI可以发现关键部位梗死、近期皮质下小梗死、腔隙、脑白质高信号、血管周围间隙扩大、微出血等VCI的特征性改变,分析这些MRI特征有利于VCI的早期诊断和治疗。采用血氧水平依赖功能磁共振成像(BOLD-fMRI)、动脉自旋标记(ASL)、动态增强MRI(DCE-MRI)、扩散张量成像(DTI)、扩散峰度成像(DKI)和7.0 T MRI等新技术可以进一步探讨VCI病理及病理生理改变,从而寻找减轻或阻碍其病理进展的机制。就MRI在VCI诊断和治疗中的研究进展予以综述。Cerebrovascular disease is the main cause of vascular cognitive impairment(VCI).The characteristic changes of VCI,such as strategic infarcts,recent small subcortical infarcts,lacunes,white matter hyperintensities,enlarged perivascular spaces,and microbleeds,can be detected on MRI.Analysis of these MRI markers facilitates early diagnosis and treatment of VCI.By using the innovative MRI techniques,such as blood oxygen level-dependent function MRI(BOLDfMRI),arterial spin labeling(ASL),dynamic contrast-enhanced MRI(DCE-MRI),diffusion tensor imaging(DTI),diffusion kurtosis imaging(DKI)and 7.0 T MRI,we can further investigate the pathogenesis and pathophysiological alterations in VCI patients,consequently could look for mechanisms to alleviate the pathology or hamper its progression.This article reviews the progress of MRI imaging in VCI diagnosis and treatment.
