摘　　要：目的探讨腱鞘纤维瘤(FTS)的MRI表现特点。方法将24例经手术、病理证实为FTS患者的MRI资料对照病理及免疫组化进行分析,总结FTS的MRI影像学特点。结果24例FTS患者病灶边界均显示清楚,在T1WI上均呈低信号,内夹杂更低信号影,与肌肉信号类似或稍低于肌肉信号。9例病灶在T2WI上呈均匀低信号,与肌腱信号类似;另15例病灶在T2WI上信号不均,边缘光滑或分叶,但边界均显示清晰。肿物由梭形和(或)星形纤维母细胞、肌纤维母细胞以及透明胶原纤维性间质组成,束状或交织状排列,核梭形或类圆形,胞浆粉染,内含数量不等的血管腔隙。免疫组化:Vim(+),SMA(+),CD34(血管+),Ki-67约5%,低表达。结论FTS大多数边界清楚且形态规则,MRI上多序列扫描均表现为低信号且增强扫描轻-中度不均匀强化等特点。Objective To investigate the MRI finding characteristics of fibroma of tendon sheath(FTS).Methods The MRI imaging data of 24 cases with FTS confirmed by surgery and pathology were analyzed,compared with pathology and immunohistochemistry,and summarized the MRI imaging characteristics of FTS.Results In all the 24 patients with FTS,the lesion boundaries were clear.The signals on T1WI were uniform or heterogenous hypointense and T1WI signals were similar to or slightly lower than the muscle signal.The signals on T2WI of 9 cases were diagnosed as low signals,similar to that of muscle tendon,the other 15 cases had uneven signals on T2WI,with smooth edges or lobules,but clear boundaries.FTS was composed of fusiform and(or)astrofibroblasts,myofibroblasts and clear collagenous stroma,arranged in fascicles or interlaced,nuclear fusiform or round,cytoplasmic powdery,and contained an unequal number of vascular lacunae.Immunohistochemistry:Vim(+),SMA(+),CD34(vascular+),Ki-67 was about 5%,with low expression.Conclusions Most of the FTS have clear boundaries and regular morphology,and the multi-sequence scanning in MRI is characterized by low signal and light to moderate non-uniform enhancement in enhanced scanning.
