摘 要:目的:探讨化学染色联合蓝激光放大内镜对食管早期癌性病变的诊断价值。方法:选取在2017年5月至2019年5月期间在我院行内窥镜检查可疑食管病变患者100例,共138处病变,本文对此100例患者分别采用蓝激光放大内镜、碘染色和蓝激光放大内镜与碘染色相结合三种方式进行进一步检查,并以病变的病理诊断为金指标,分别对三种检查方式的镜下诊断结果与病理诊断结果行Kappa一致性检验,配对卡方的McNemar检验,采用灵敏度、特异度、阳性预测值、阴性预测值、Youden指数进行真实性评价,比较3种内镜检查方式对食管早期癌性病变的镜下诊断与病理诊断是否有统计学差异。结果:蓝激光放大与碘染色相结合组一致性好,灵敏度、特异度、阳性预测值、阴性预测值、Youden指数高。结论:蓝激光放大与碘染色相结合能提高早期食管癌性病变的诊断率。Objective:To investigate the value of chemical staining combined with blue laser magnifying endoscopy in the diagnosis of early esophageal carcinoma and precancerous lesions.Methods:From May 2017 to May 2019,100 patients with 138 suspicious esophageal lesions were examined by endoscopy in our hospital.The 100 patients were examined by blue laser magnifying endoscopy,iodine staining and blue laser magnifying endoscopy combined with iodine staining,and the pathological diagnosis was taken as the gold index.Kappa consistency test was performed for the microscopic and pathological results of the three methods,and the reliability of the McNemar chi-squartest was evaluated by sensitivity,specificity,positive predictive value,negative predictive value and Youden Index.Results:The combination of blue laser amplification and iodine staining showed good consistency,high sensitivity,specificity,positive predictive value,negative predictive value and Youden index.Conclusion:The combination of blue laser amplification and iodine staining can improve the diagnosis rate of early esophageal carcinoma.
