摘 要:目的通过高分辨磁共振技术(High-resolution MRI,HR-MRI)对大脑中动脉(Middle cerebral artery,MCA)斑块的分布、斑块的稳定性、斑块的类型及血管壁的厚度进行分析测定,从而为动脉狭窄引起的急性缺血性脑卒中的早期诊断及临床治疗提供依据。方法选取2017年4月—2019年3月在本院进行常规磁共振成像(MRI)扫描并诊断为脑梗死的152例患者作为研究对象,根据颅内动脉粥样硬化性血管有无狭窄,分为狭窄组和非狭窄组两组,每组各76例。使用HR-MRI对两组患者MCA血管进行分析,并对MCA斑块的位置分布、斑块的稳定性、斑块的分型和血管壁厚度进行分析测定。结果狭窄组患者的不稳定斑块的数量要显著多于非狭窄组患者(χ^2=4.193,P=0.047)。对两组患者的斑块分型的判定显示,狭窄组患者的斑块主要是不稳定的软斑的,数量为36块;而非狭窄组患者斑块的类型主要是稳定的硬斑,数量为34块。对血管壁厚度分析结果显示,狭窄组的血管壁厚度为(1.95±0.19)mm,而非狭窄组为(0.92±0.15)mm。结论通过HR-MRI技术能够对MCA的斑块位置、斑块的稳定性、斑块的分型和血管壁的厚度进行分析测定,因此可以为卒中等脑血管疾病的诊断、治疗、预后以及疾病的发生机制提供新的思路。Objective To determine the distribution, stability and type and thickness of plaque middle cerebral artery(MCA)by high-resolution MRI(HR-MRI), in order to provide references for early diagnosis and clinical treatment for acute ischemic stroke caused by artery stenosis.Methods 152 patients those received routine magentic resonance imaging(MRI) scanning and diagnosed as cerebral infarction in the hospital from April 2017 to March 2019 were enrolled as study subjects, they were divided into stenosis group and non-stenosis group according to the presence or absence of intracranial atherosclerotic vessels stenosis,76 cases in each group. Then HR-MRI was used to analyze MCA vessels, and the plaque location distribution, plaque stability, plaque typing and vascular wall thickness were analyzed and measured.Results The number of unstable plaques in the stenosis group was significantly higher than that in the non-stenosis group(χ^2=4.193,P=0.047). The plaque types of patients in the stenosis group were mainly unstable soft plaques(the number is 36), while those in the non-stenosis group were mainly stable hard plaques(the number is 34). The thickness of the vascular wall of the stenosis group was(1.95±0.19) mm, while that of the non-stenosis group was(0.92±0.15) mm.Conclusions HR-MRI technology cold be used to analyze and measure the plaque location, plaque stability, plaque typing and vascular wall thickness of MCA, thus providing new ideas for the diagnosis, treatment, prognosis and pathogenesis of stroke and other cerebrovascular diseases.
