经直肠超声引导下前列腺穿刺活检联合前列腺抗原在诊断前列腺癌中的价值

摘　　要：目的评估经直肠超声引导下前列腺穿刺活检联合前列腺抗原在诊断前列腺癌中的价值。方法选取60例本院2017年6月至2018年6月收治的前列腺癌患者,并根据随机数字法分为三组,A组单独给予经直肠超声引导下前列腺穿刺活检,B组采用前列腺抗原诊断,C组采用两种方法联合诊断。观察三组的检出率、患者的分期情况、不同分期患者的PSA检查结果。结果 C组前列腺癌检出率95%、误诊率0%、漏诊率5%,与A组与B组对比差异显著(P <0.05)。C组患者A期、B期检出率分别为30%、45%,与A组及B组对比差异显著(P <0.05)。三组C期与D期检出率,无显著差异(P> 0.05)。C组患者A期、B期、C期及D期PSA分别为(5.9±0.3)μg/L、(14.5±1.2)μg/L、(30.1±4.8)μg/L及(50.2±6.2)μg/L;A期、B期、C期及D期PSA阳性率分别为16.7%、40%、80%及100%,A组、B组、C组PSA阳性率对比无显著差异(P> 0.05)。结论采用经直肠超声引导下前列腺穿刺活检联合前列腺抗原诊断前列腺癌,能够提高检出率,为疾病的治疗提供指导。Objective To evaluate the value of transrectal ultrasound-guided prostate biopsy combined with prostate antigen in the diagnosis of prostate cancer.Methods A total of 60 patients with prostate cancer admitted from June 2017 to June 2018 were divided into three groups according to the random number method,group A received transrectal ultrasound-guided prostate biopsy alone,group B was diagnosed by prostate antigen,group C was diagnosed by two methods.The detection rate of three groups,the stage of patients,patients with different stages of PSA test results were observed.Results The detection rate of prostate cancer in group C was 95%,the misdiagnosis rate was 0%and the missed diagnosis rate was 5%,the difference was significant(P<0.05).The positive rates of stage A and stage B in group C were 30%and 45%respectively,which were significantly different from those in group A and group B(P<0.05).There was no significant difference in phase C and phase D between the three groups(P>0.05).The PSA in phase A,phase B,phase C and phase D were(5.9±0.3)μg/L,(14.5±1.2)μg/L,(30.1±4.8)μg/L and(50.2±6.2)μg/L,respectively the positive rates of PSA in stage A,B,C and D were 16.7%,40%,80%and 100%,respectively.There was no significant difference between group A,B and C(P>0.05).Conclusion Transrectal ultrasound guided prostate biopsy combined with prostate antigen diagnosis of prostate cancer can improve the detection rate and provide guidance for the treatment of disease.
