超声造影征象及免疫因子与三阴性乳腺癌的相关性研究

摘　　要：目的分析三阴性乳腺癌(TNBC)与非三阴性乳腺癌(NTNBC)的声像图以及超声造影征象特征,探讨TNBC的超声及造影表现与相关免疫因子中抑癌基因P53、P63及角细胞蛋白CK5/6、雄激素受体(AR)的表达水平的相关性。方法收集2015年1月-2019年6月于新疆医科大学附属肿瘤医院乳腺外科接受乳腺癌手术并术后病理证实为TNBC的72例患者,以及125例NTNBC患者的临床资料。采用二维超声及超声造影检查收集TNBC及NTNBC患者乳腺癌结节的大小、形态、边缘、边界、后方回声、有无微钙化、血流、淋巴结有无转移,造影增强程度、造影时穿支血管、造影后结节大小,分析TNBC的超声及造影表现与CK5/6、P53、P63、AR表达水平的相关性。结果与NTNBC组比较,TNBC结节在二维超声上更多形态为圆形或者椭圆形,边缘清晰,后方回声增强,无微钙化,淋巴结转移少,差异有统计学意义(P<0.05)。超声造影中多为高增强,造影前后结节大小无明显变化,差异有统计学意义(P<0.05)。二维超声及造影征象均与AR、P63的表达无相关性(P>0.05)。边界不清,内部有微钙化,淋巴结转移,造影前后结节测量值增大与CK5/6表达阳性具有相关性,相关系数为0.285、0.568、0.326、0.369,差异有统计学意义(P<0.05)。血流丰富、造影呈高增强、造影有穿支血流灌注与P53表达阳性有关,相关系数分别为0.416、0.275、0.320,差异有统计学意义(P<0.05)。结论TNBC的超声以及造影的征象与CK5/6、P53的阳性表达有一定的相关性。Objective To analyze the sonographic and contrast-enhanced features of triple-negative breast cancer(TNBC)and non-three-negative breast cancer(NTNBC).Analyze the relationship between image characteristics and the expression of tumor suppressor genesP53,P63 and CK5/6 of Cytkoratin,androgen receptor(AR)in TNBC.Methods To collect the clinical data of 72 patients,which undergoing breast cancer surgery with postoperative pathological TNBC confirmed,and 125 NTNBC patients randomly selected,in the department of breast surgery affiliated Cancer Hospital of Xinjiang Medical University from January 2015 to June 2019.The size,shape,edge,boundary,posterior echo,microcalcification,blood flow,lymph node metastasis,contrast enhancement,perforating vessels and nodule size after contrast-enhanced are collected of TNBC and NTNBC,by two-dimensional ultrasound and contrast-enhanced ultrasound examination.Retrospective analysis of the correlation between TNBC ultrasound and contrast-enhanced ultrasound features with CK5/6,P53,P63,AR expression levels.Results Compared with ntnbc group,TNBC nodules were more round or oval in shape,with clear edge,enhanced posterior echo,no microcalcification and less lymph node metastasis(P<0.05).There was no significant change in the size of nodules before and after contrast-enhanced ultrasound(P<0.05).There was no correlation between the expression of AR and p63(P>0.05).The positive expression of CK5/6 was correlated with unclear boundary,microcalcification,lymph node metastasis,and increased nodule measurement before and after angiography,and the correlation coefficients were 0.285,0.568,0.326,0.369,and the difference was statistically significant(P<0.05).The correlation coefficients were 0.416,0.275 and 0.320 respectively(P<0.05).Conclusion TNBC ultrasound and contrast-enhanced signs and CK5/6,p53 positive expression has a certain correlation.
