恩度联合放、化疗对中晚期宫颈癌疗效及CD4^+T细胞平衡失调的影响

摘　　要：目的探讨恩度联合放、化疗对中晚期宫颈癌疗效及CD4^+T细胞平衡失调的影响。方法选择我院收治的116例中晚期宫颈癌患者为研究对象,其中观察组60例,采用恩度+紫杉醇、顺铂(TP)周疗+同步放疗,对照组56例,采用TP周疗+同步放疗。观察两组患者的近期疗效,并比较辅助性T细胞1(Th1)、辅助性T细胞2(Th2)、辅助性T细胞17(Th17)、调节性T细胞(Treg)百分率、血清细胞因子水平及不良反应发生率。结果观察组治疗总有效率为93.33%,略高于对照组的83.93%,但差异无统计学意义(P>0.05)。治疗后,两组患者Th1百分率及Th1/Th2均显著高于治疗前(P<0.05),且观察组显著高于对照组(P<0.05)。治疗后,对照组Th2百分率与治疗前无明显差异(P>0.05),观察组Th2百分率显著低于治疗前(P<0.05),且明显低于对照组(P<0.05)。治疗后,两组患者Th17、Treg百分率及叉头蛋白核转录因子(FoxP3)、白细胞介素-4(IL-4)、白细胞介素-17(IL-17)水平均显著低于治疗前(P<0.05),且观察组显著低于对照组(P<0.05);Th17/Treg、血清干扰素-γ(IFN-γ)水平均显著高于治疗前(P<0.05),且观察组显著高于对照组(P<0.05)。治疗期间两组患者胃肠道反应、放射性直肠反应、骨髓抑制、肝肾功能异常等不良反应的发生率比较,差异均无统计学意义(P>0.05)。结论恩度联合放、化疗治疗晚期宫颈癌可改善患者CD4^+T细胞失衡状态,发挥抗肿瘤作用。Objective To investigate the efficacy of recombinant human endostatin(Endostar)combined with radiotherapy and chemotherapy on advanced cervical cancer and its effects on imbalance of CD4^+T cells of patients.Methods A total of 116 patients with advanced cervical cancer in our hospital were selected as the research objects.Of them,60 patients were treated with Endostar+paclitaxel and cisplatin(TP)weekly therapy+concurrent radiotherapy,and were taken as the observation group.The other 56 patients received TP weekly therapy+concurrent radiotherapy,and were taken as the control group.Observe the clinical efficacy of the two groups,and compare the percentages of Th1,Th2,Th17 and regulatory T cells(Treg),serum cytokines,and recent adverse reactions.Results The total effective rate in the observation group was 93.33%,which was slightly higher than 83.93%in the control group,but there was no significant difference between the two groups(P>0.05).The percentage of Th1 and Th1/Th2 in the two groups after treatment were higher than those before treatment(P<0.05).After treatment,the percentage of Th1 and Th1/Th2 were higher in observation group than in control group(P<0.05).There was no difference in Th2 percentage before and after treatment in the control group(P>0.05).But the percentage of Th2 in the observation group after treatment was lower than that before treatment,and was lower than that in the control group after treatment(P<0.05).After treatment,the percentages of Th17 and Treg,and the levels of forkhead box protein 3(FoxP3),interleukin-4(IL-4),and interleukin-17(IL-17)in the two groups were lower than those before treatment(P<0.05),but the Th17/Treg ratio and serum interferon-γ(IFN-γ)level were higher than those before treatment(P<0.05).The other indexes of the observation group were better than those of the control group(P<0.05).No significant differences were found between the two groups in adverse reactions such as gastrointestinal reaction,radiation rectal reaction,myelosuppression,liver and kidney dysfunctio
