摘　　要：目的分析CYP2D6基因分型与羟考酮给药间期末疼痛(EDF)的相关性,以期为临床个体化镇痛提供依据。方法采集患者静脉血,提取血液基因组DNA,PCR检测CYP2D6基因分型,数字分级评分法对患者疼痛程度进行评估,比较不同基因分型患者的羟考酮镇痛效果和EDF的差异。结果共纳入服用羟考酮的癌痛患者64例,EDF发生率为4.69%,其中CYP2D6正常代谢型(NM)42例、慢代谢型(PM)22例,未检测到超快代谢型(UM)和中间代谢型(IM)。NM与PM组患者的羟考酮镇痛效果和EDF发生率均无显著差异(P>0.05)。结论CYP2D6正常代谢型和慢代谢型与羟考酮EDF无明显相关性,但超快代谢型和中间代谢型对患者EDF的影响及最终结论尚需扩大样本量进一步研究。Objective To assess the relationship between CYP2D6 genetic polymorphisms and end-of-dose failure pain(EDF)with oxycodone in patients with cancer pain,so as to provide experiment basis for clinical individual analgesia.Methods Patients’venous blood was collected to extract genomic DNA,and the CYP2D6 genotype was analyzed by PCR method.The pain degrees were assessed by using the numerical rating scale.Compare the analgesic effects of oxycodone and EDF between patients with different genotypes.Results A total of 64 cancer pain patients taking oxycodone were included.The incidence of EDF in oxycodone analgesia was 4.69%.The samples consisted of 42 normal metabolizers(NM)and 22 poor metabolizers(PM).No ultrarapid metabolizers(UM)or intermediate metabolizers(IM)were observed.And there was no statistical difference between the NM and PM groups in pain control and EDF.Conclusion CYP2D6 NM and PM did not influence end-of-dose failure pain.However,the effects of CYP2D6 UM and IM on EDF and the final conclusions need to be further studied in large samples.
