摘　　要：目的研究B7-H4(B7同源物4)蛋白在胆囊腺瘤恶变为胆囊腺癌过程中的意义。方法选取河南大学第一附属医院2005年6月至2015年6月符合条件胆囊腺癌病例共58例,采用免疫组织化学法检测B7-H4在胆囊腺瘤和胆囊腺癌中的表达。应用SPSS 17.O软件包对其表达分析,所有结果α=0.05(双侧)检验水准。结果B7-H4蛋白在胆囊腺瘤阳性表达率为15.00%(6/40),在胆囊腺癌阳性表达率为67.24%(39/58),差异有统计学意义(χ~2=26.017,P<0.05)。B7-H4蛋白在胆囊腺癌低-中分化组阳性表达率为69.44%(25/36),在高分化组的阳性表达率为33.33%(4/12),差异有统计学意义(χ~2=4.907,P=0.027)。B7-H4在胆囊腺癌TNM(Ⅰ+Ⅱ)和TNM(Ⅲ+Ⅳ)中表达差异无统计学意义(χ~2=2.379,P=0.123)。B7-H4阳性表达与病人的年龄和性别无相关(χ~2=0.005,P=0.944),与伴胆囊结石(χ~2=7.608,P=0.006)、肿瘤长径(χ~2=6.522,P=0.011)、淋巴结转移(χ~2=3.928,P=0.047)相关。Kaplan-Meier生存分析:B7-H4阳性表达组中位生存率为26月,95%CI:20.661~31.339,B7-H4蛋白阴性表达组中位生存期为22月,95%CI:6.359~37.641,Log-Rank(Mantel-Cox)显示:B7-H4蛋白表达阳性者与表达阴性者三年生存期差异无统计学意义(χ~2=0.187,P=0.665)。结论B7-H4蛋白在胆囊腺癌中表达明显高于胆囊腺瘤。B7-H4蛋白的阳性表达与胆囊腺癌的分化程度、肿瘤长径、胆囊结石及淋巴结转移相关,与胆囊病理分期、年龄、性别无相关。Objective To explore the significance of B7-H4 in the process of gallbladder adenoma becoming gallbladder adenocarcinoma.Methods A total of 58 eligible cases of gallbladder adenocarcinoma from June 2005 to June 2015 in the First Affiliated Hospital of Henan University were selected,and the expression of B7-H4 in gallbladder adenocarcinoma and gallbladder adenoma was detected by immunohistochemistry.SPSS17.0 software package was used to analyze its expression,all resultsα=0.05(bilateral)test level,P<0.05 was statistically significant.Results The positive expression rate of B7-H4 protein in gallbladder adenoma was 15.00%(6/40),and the positive expression rate in gallbladder adenocarcinoma was 67.24%(39/58),the difference was statistically significant(χ~2=26.017,P<0.05).The positive expression rate of B7-H4 protein in the low-medium differentiated group of gallbladder adenocarcinoma was 69.44%(25/36),and the positive expression rate in the well-differentiated group was 33.33%(4/12),the difference was statistically significant(χ~2=4.907,P=0.027).There was no significant difference in the expression of B7-H4 in gallbladder adenocarcinoma TNM(Ⅰ+Ⅱ)and TNM(Ⅲ+Ⅳ)(χ~2=2.379,P=0.123).The positive expression of B7-H4 was not correlated with the age and gender(χ~2=0.005,P=0.944),but was correlated with gallbladder stones(χ~2=7.608,P=0.006),tumor diameter(χ~2=6.522,P=0.011),lymph nodes Metastasis(χ~2=3.928,P=0.047).Kaplan-Meier survival analysis:The median survival rate of the B7-H4 positive expression group was 26 months,95%CI:20.661-31.339,the median survival time of the B7-H4 protein negative expression group was 22 months,95%CI:6.359-37.641,Log-Rank(Mantel-Cox)showed that there was no significant difference in the three-year survival between B7-H4 protein expression positive and negative expression(χ~2=0.187,P=0.665).Conclusions The expression of B7-H4 protein in gallbladder adenocarcinoma is significantly higher than that in gallbladder adenoma.The positive expression of B7-H4 protein is associated with th
