摘　　要：目的分析女性前交通动脉瘤破裂的相关危险因素,旨在为该病的预防提供参考。方法回顾性分析2014年6月~2020年6月连续来连云港市第一人民医院就诊的女性前交通动脉瘤患者106例为研究对象,根据动脉瘤是否破裂分为破裂组82例,未破裂组24例。收集患者的年龄、身高体重指数(BMI)、高血压史、吸烟史、饮酒史、糖尿病史并比较,通过西门子后处理工作站测出动脉瘤的高度、瘤颈宽度,并计算瘤颈比(AR)值。Logistic回归分析女性前交通动脉瘤破裂的相关危险因素,并用ROC曲线计算最佳诊断阈值。结果两组年龄、BMI、吸烟史、饮酒史、糖尿病史比较,差异无统计学意义(P>0.05)。两组高血压病史、动脉瘤的高度、瘤颈宽度、AR值、形态是否规则比较(P<0.05);多因素回归分析显示动脉瘤形态和AR值是女性前交通动脉瘤破裂风险的危险因素(P<0.05),当AR值>1.6时,诊断动脉瘤破裂风险的敏感度为86.40%,特异度为80.60%。结论瘤体形态不规则和AR值>1.6时女性前交通动脉瘤患者,其瘤体更容易发生破裂。Objective The analysis of the risk factors related to the rupture of female anterior communicating artery aneurysm aims to provide reference for the prevention of the disease.Methods A retrospective analysis of 106 female patients with anterior communicating aneurysm who visited Lianyungang First People's Hospital from June 2014 to June 2020 was the research object.According to whether the aneurysm was ruptured,they were divided into 82 cases in the ruptured group and 24 cases in the unruptured group.Collecting the patient's age,height and body mass index(BMI),history of hypertension,history of smoking,history of drinking,history of diabetes and compare them,and measure the height of the aneurysm and the width of the aneurysm through the Siemens post-processing workstation and calculate the tumor neck ratio(AR)value.Logistic regression analysis of risk factors related to rupture of female anterior communicating artery aneurysm.Finally,the optimal diagnostic cutoff values were calculated using ROC curves.Results There was no significant difference in age,BMI,smoking history,drinking history,and diabetes history between the two groups(P>0.05).The two groups of hypertension history,height of aneurysm,width of aneurysm neck,AR value,morphology are compared(P<0.05);multivariate regression analysis showed that the aneurysm morphology and AR value are female anterior communicating arteries Risk factors for the risk of tumor rupture(P<0.05).When the AR value was>1.6,the sensitivity and specificity of aneurysm rupture risk diagnosis were 86.40% and 80.60% respectively.Conclusion Female patients with anterior communicating aneurysm with irregular tumor shape and AR value>1.6 are more likely to rupture.
