开放性眼外伤后合并眼内炎危险因素分析

摘　　要：目的分析开放性眼外伤后眼内炎发生(Post-traumatic endophthalmitis,PTE)的危险因素,为临床预防及早期诊断、治疗PTE提供依据。方法选取2017年至2019年在乐山市人民医院眼科就诊的356名开放性眼外伤患者,将患者是否发生眼内炎分为两组。发生眼内炎的为病例组,共20例,未发生眼内炎的分为对照组,共336例。收集患者的年龄、视力、受伤部位(角膜、角膜缘、巩膜)、受伤地点(农村、城市)、受伤到医治的时间、玻璃体是否脱出以及眼内是否存留异物等,分析上述因素与PTE发生的相关性。结果在选取的356名开放性眼外伤患者中有20名发生眼内炎,开放性眼外伤后眼内炎发生率为5.6%。经统计学分析发现,患者受伤年龄越小、受伤地点为农村、受伤到医治大于24小时为开放性眼外伤后发生眼内炎的危险因素(P<0.05)。结论对开放性眼外伤患者,受伤年龄越小、受伤地点为农村、受伤到医治的时间大于24小时为PTE发生的危险因素。Objective To analyze the risk factors of post traumatic endophthalmitis(PTE),and to provide evidence for clinical prevention and early diagnosis and treatment of PTE.Methods From 2017 to 2019,356 patients with open globe injury who were treated in ophthalmology department of Leshan people’s hospital were divided into two groups.The patients with endophthalmitis were divided into case group(n=20)and The patients without endophthalmitis were divided into control group(n=336).Age,visual acuity,injury sites(cornea,limbus,sclera),injury location(rural,urban),duration from injury to treatment,condition of vitreous body,whether there is foreign body in the eye and other indicators were collected as analysis objects,and the correlation with PTE was analyzed.Results Among the 356 patients with open eye injury,20 patients had endophthalmitis,and the incidence of post-traumatic endophthalmitis was 5.6%.Statistical analysis showed that the younger the age of injury,the place of injury in rural areas,and the time from injury to treatment more than 24 hours were the risk factors of post-traumatic endophthalmitis(P<0.05).Conclusion for patients with open ocular trauma,the younger age group,cases from rural area,and the time before treatment longer than 24 hours were the risk factors for the occurrence of PTE.
