川芎嗪注射液联合依达拉奉治疗颅内动脉瘤术后脑血管痉挛的效果

摘　　要：目的探讨川芎嗪注射液联合依达拉奉治疗颅内动脉瘤术后脑血管痉挛的临床效果。方法选取2017年1月—2019年12月内蒙古赤峰市医院收治的100例颅内动脉瘤术后脑血管痉挛患者,将其按照随机数字表法分为对照组和研究组,每组各50例。对照组予以依达拉奉治疗,研究组在对照组的基础上联合川芎嗪注射液治疗。比较两组疗效、神经功能、大脑中动脉血流速度(VMCA)、血清相关指标及不良反应。结果研究组治疗4周后的临床总有效率高于对照组(P<0.05)。两组治疗4周后美国国立卫生研究院卒中量表(NIHSS)评分、血浆白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、内皮素-1(ET-1)水平较治疗前降低,且研究组低于对照组(P<0.05)。两组治疗4周后一氧化氮(NO)、VMCA高于治疗前,且研究组高于对照组(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论依达拉奉联合川芎嗪注射液治疗颅内动脉瘤术后脑血管痉挛,效果显著,可促进机体神经功能改善,恢复血流速度,且安全可靠,可能与改善机体IL-6、TNF-α、ET-1和NO水平有关。Objective To investigate the clinical effect of Ligustrazine Injection combined with Edaravone in the treatment of cerebral vasospasm after intracranial aneurysm surgery.Methods A total of 100 patients with cerebral vasospasm after intracranial aneurysm surgery were selected from Chifeng Municipal Hospital,Inner Mongolia Autonomous Region from January 2017 to December 2019.They were divided into control group and study group according to the random number table method,with 50 cases in each group.The control group was treated with Edaravone and the study group was treated with Ligustrazine Injection on the basis of the control group.The efficacy,nervous function,middle cerebral artery blood flow velocity(VMCA),serum related indexes and adverse reactions were compared between the two groups.Results After four weeks of treatment,the clinical total effective rate of the study group was higher than that of the control group(P<0.05).After four weeks of treatment,the score of national Institutes of health stroke scale(NIHSS),the levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),endothelin-1(ET-1)in the two groups were lower than those before treatment,and the study group was lower than the control group(P<0.05).After four weeks of treatment,the levels of nitric oxide(NO)and VMCA in the two groups were higher than those before treatment(P<0.05),and the study group was higher than that in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Ligustrazine Injection combined with Edaravone is effective in the treatment of cerebral vasospasm after intracranial aneurysm surgery.It can promote the improvement of the body’s nervous function,restore blood flow speed,and it is safe and reliable.It may be related to the improvement of the body’s IL-6,TNF-α,ET-1 and NO levels.
