摘　　要：目的分析不同浓度5-氨基酮戊酸(5-ALA)光动力疗法治疗宫颈上皮内瘤变(CIN)1/2伴高危型人乳头状瘤病毒(HR-HPV)感染患者的临床效果。方法选取2017年1月—2019年6月江苏省宜兴市人民医院就诊的80例CIN1/2伴HR-HPV感染患者为研究对象,按照随机数字表法将其分为观察组和对照组,每组40例。对照组给予低浓度5-ALA,观察组采用高浓度5-ALA治疗,比较两组治疗后3、6个月宫颈轻度异常或正常人数、HR-HPV转阴率、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平及不良反应发生情况。结果治疗后3、6个月,两组CIN1患者宫颈轻度异常或正常例数及HR-HPV DNA转阴率比较,差异无统计学意义(P>0.05);观察组CIN2患者宫颈轻度异常或正常例数、HR-HPV DNA转阴率高于对照组CIN2患者(P<0.05)。治疗后3个月,两组IL-6水平高于治疗前、TNF-α水平低于治疗前;观察组IL-6水平高于对照组,TNF-α水平低于对照组(均P<0.05)。所有患者均未出现继发感染、出血、宫颈管狭窄或瘢痕形成。结论高浓度5-ALA光动力疗法治疗CIN1/2伴HR-HPV感染患者的临床效果显著,可有效提高HR-HPV转阴率,同时调节炎症因子水平,提高患者细胞免疫水平。Objective To analyze the efficacy of photodynamic therapy with different concentrations of 5-aminolevulinic acid(5-ALA)in the treatment of cervical intraepithelial neoplasia(CIN)1/2 patients with high risk of human papillomavirus(HR-HPV)infection.Methods A total of 80 cases of CIN1/2 patients with HR-HPV infection who treated in Yixing People’s Hospital of Jiangsu Province from January 2017 to June 2019 were selected as the study objects.They were divided into the observation group and the control group according to random number table method,with 40 cases in each group.The control group was given low-concentration 5-ALA,and the observation group was treated with high-concentration 5-ALA.The number of patients with mild cervical abnormalities or normal cervical,HR-HPV negative conversion rate,interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)levels and adverse reactions at 3 and 6 months after treatment were compared between the two groups.Results There were no statistically significant differences between the two groups in the number of mild cervical abnormalities or normal cervical and HR-HPV DNA conversion negative rate of CIN1 patients between the two groups at 3 and 6 months after treatment(P>0.05);the number of mild cervical abnormalities or normal cervical and HR-HPV DNA conversion negative rate of CIN2 patients in the observation group were higher than those in the control group(P<0.05).IL-6 level at 3 months after treatment was higher than that before treatment,and TNF-αlevel was lower than that before treatment;the levels of IL-6 and TNF-αin the observation group were higher than those in the control group(all P<0.05).None of the patients had secondary infection,bleeding,cervical stenosis or scar formation.Conclusion High concentration 5-ALA photodynamic therapy for CIN1/2 patients with HR-HPV infection has a significant clinical effect,which can effectively improve the negative rate of HR-HPV,and improve the level of inflammatory factors.
