摘　　要：目的探讨脂蛋白相关磷脂酶A2(Lp-PLA2)、胰岛素样生长因子-1(IGF-1)与老年血管性痴呆(VD)患者认知功能的关系及预测价值。方法选择2019年3~6月在我院接受治疗的VD患者60例为研究对象,同时选取同期在我院接受检查的健康者60例为对照组,比较两组患者的Lp-PLA2、IGF-1、简易智力状态检查量表评分(MMSE)水平;采用Pearson相关性分析法对Lp-PLA2、IGF-1与MMSE评分的相关性进行分析;采用受试者工作曲线(ROC)下面积(AUC)比较Lp-PLA2、IGF-1对VD的预测价值。结果观察组患者的Lp-PLA2水平明显高于对照组(P<0.05),观察组患者的IGF-1、MMSE评分明显低于对照组(P<0.05);Lp-PLA2与MMSE评分呈负相关性,IGF-1与MMSE评分呈正相关性;Lp-PLA2与IGF-1诊断VD的AUC值分别为0.940、0.954。结论Lp-PLA2水平越低,IGF-1水平越高,患者的认知功能越好,且Lp-PLA2、IGF-1对老年VD患者均具有较高的预测价值。Objective To explore the relationship and predictive value of lipoprotein-associated phospholipase A2(Lp-PLA2),insulin-like growth factor-1(IGF-1)and cognitive function in elderly patients with vascular dementia(VD).Methods A total of 60 cases of VD patients who were treated in our hospital from March to June 2019 were selected as the study subjects,while 60 healthy people who were examined in our hospital during the same period were selected as the control group.And the levels of Lp-PLA2,IGF-1,and mini-mental state examination scale score(MMSE)in the two groups were compared.Pearson correlation analysis was used to analyze the correlation between Lp-PLA2,IGF-1 and MMSE score;the predictive value of Lp-PLA2 and IGF-1 on VD was compared using the area under the receiver(AUC)operating curve(ROC).Results The levels of Lp-PLA2 in the observation group were significantly higher than those in the control group(P<0.05),and the IGF-1 and MMSE scores in the observation group were significantly lower than those in the control group(P<0.05);Lp-PLA2 and MMSE score were negatively correlated,and IGF-1 and MMSE scores were positively correlated;the AUC values of Lp-PLA2 and IGF-1 for diagnosis of VD were 0.940 and 0.954,respectively.Conclusion The lower the level of Lp-PLA2,the higher the level of IGF-1,and the better the cognitive function of the patient.Lp-PLA2 and IGF-1 have higher predictive value for elderly VD patients.
