摘 要:目的观察光学相干生物测量仪(IOLMaster)与A超检查对白内障术前测量的影响。方法病例来源于我院2019年1月~2020年1月收治的白内障患者共392例(706眼),采用随机数字表法将其分为观察组(n=197,355眼)与对照组(n=195,351眼),对照组行A超检查,观察组行IOLMaster检查,观察两种检查方式对白内障术前测量的影响。结果IOLMaster测量的眼轴长度值长于A超测量值(P<0.05),IOLMaster测量的前房深度值长于A超测量值(P<0.05);在眼轴长度20.00~22.00 mm、22.10~24.50 mm、24.60~26.00 mm中,IOLMaster测量眼轴长度值与接触式A超测量结果比较,差异无统计学意义(P>0.05),在眼轴长度值及>26.10 mm中,IOLMaster测量眼轴长度值长于接触式A超测量眼轴长度值(P<0.05);术后3个月MAE≤0.50 D、≤1.00 D、≤1.50 D、≤2.00 D时,IOLMaster检查MAE高于接触式A超检查(P<0.05),MAE>2.00 D时,IOLMaster检查MAE低于接触式A超检查(P<0.05)。结论与接触式A超比较,白内障术前测量采用IOLMaster检查的准确性更高,IOL度数测量更精确,可为临床选择测量方式提供参考。Objective To observe the impacts of IOLMaster and A-scan examination in preoperative measurement of cataract.Methods A total of 392 cataract patients(706 eyes)admitted to our hospital from January 2019 to January 2020 were selected.They were divided into the observation group(n=197,355 eyes)and the control group(n=195,351 eyes)according to the random number table method.A-scan examination was performed in the control group,while IOLMaster examination was performed in the observation group.The effects of the two methods on preoperative measurement of cataract were observed.Results The length of eye axis measured by IOLMaster was longer than that measured by A-scan(P<0.05),while the anterior chamber depth measured by IOLMaster was longer than that measured by A-scan(P<0.05).There were no differences between IOLMaster and contact-type A-scan in the axial length of 20.00-22.00 mm,22.10-24.50 mm and 24.60-26.00 mm(P>0.05).When the eye axis length>26.10 mm,the eye axis length measured by IOLMaster was longer than that measured by contact-type A-scan(P<0.05).At 3 months after operation,when MAE≤0.50 D,≤1.00 D,≤1.50 D,≤2.00 D,the MAE of IOLMaster examination was higher than that of contact-type A-mode ultrasonic examination(P<0.05),and when MAE>2.00 D,the MAE of IOLMaster examination was lower than that of contact-type A-mode ultrasonic examination(P<0.05).Conclusion Compared with contact-type A-scan,IOLMaster is more accurate in preoperative measurement of cataract,and IOL degree measurement is more accurate,which provides reference for clinical selection of measurement methods.
