奥氮平预防化疗所致恶心呕吐的研究进展

摘　　要：5-羟色胺3受体拮抗剂(5-HT3 RA)、神经激肽1受体拮抗剂(NK-1 RA)、地塞米松是常用的化疗止吐药物,三者的联合方案受到国际止吐指南推荐。而上述止吐方案仍不能完全控制化疗所致的恶心呕吐(CINV)。2017年2月,美国国家综合癌症网络首次将联合奥氮平(olanzapine)与上述三药的四联方案列入NCCN止吐指南,奥氮平控制CINV延迟期恶心更有优势,临床研究推荐低剂量5 mg/d。奥氮平因其良好的有效性、安全性、经济性受到国内外关注,现对奥氮平防治CINV的进展作一综述。5-hydroxytryptamine 3 receptor antagonists(5-HT3 RA),neurokinin-1 receptor antagonists(NK-1 RA),dexamethasone are commonly used chemotherapy antiemetic drugs.Multinational antiemetic guidelines recommend the combination of the triple regimens.However,the controlling of chemotherapy-induced nausea and vomiting(CINV)is undesirable.In February 2017,olanzapine combined with the adove triple prevention regimens is included in the antiemetic guidelines by NCCN.Clinical studies have confirmed that the low-dose olanzapine 5 mg/day combination regimen has a good efficacy in the prevention of CINV,especially the delayed nausea.Olanzapine has been paid more attention at home and abroad because of its good effectiveness,safety and economy.The progress of olanzapine in preventing CINV is reviewed.
