半夏泻心汤联合三联疗法治疗Hp相关性消化性溃疡有效性及安全性的Meta分析

摘　　要：[目的]对半夏泻心汤联合三联疗法治疗Hp相关性消化性溃疡(peptic ulcer,PU)的有效性以及安全性进行评估。[方法]通过检索知网、万方、维普、CBM、PubMed、Cochrane library等数据库,纳入半夏泻心汤联合三联疗法治疗Hp相关性PU的随机对照临床研究,筛选文献并提取相关数据,利用RevMan5.3软件进行Meta分析。[结果]24篇相关文献纳入本研究,共计2172例患者,Meta分析结论示:临床总有效率(RR=1.23,95%CI1.18~1.27,P<0.00001);临床治愈率(RR=1.54,95%CI1.37~1.72,P<0.00001);Hp根除率(RR=1.25,95%CI 1.19~1.30,P<0.00001);1年后溃疡复发率(RR=0.17,95%CI0.02~1.33,P=0.09);不良反应发生率(RR=0.35,95%CI0.23~0.53,P<0.00001)。[结论]与单纯使用三联疗法相比,联合使用经方半夏泻心汤能够有效提高临床治疗Hp相关性PU的有效性和安全性,但受纳入研究质量的影响,本研究结论尚需更多高质量的RCT试验进一步验证。[Objective]To evaluate the efficacy and safety of Banxia Xiexin decoction combined with triple therapy in the treatment of Helicobacter pylori(Hp)-related peptic ulcer(PU).[Methods]the randomized controlled clinical trials of Banxia Xiexin decoction combined with triple therapy in the treatment of Hp related PU were included by searching CNKI,Wanfang,VIP,CBM,PubMed,Cochrane Library and other databases.The literatures were screened and the relevant data were extracted.The meta-analysis was conducted by using the 5.3software.[Results]Twenty four related literatures were included in this study,with a total of 2172patients included in the meta-analysis.conclusion:the clinical total effective rate(RR=1.23,95%CI 1.18~1.27,P<0.00001);clinical cure rate(RR=1.54,95%CI 1.37~1.72,P<0.00001);Hp eradication rate(RR=1.25,95%CI 1.19~1.30,P<0.00001);ulcer recurrence rate after one year(RR=0.17,95%CI 0.02~1.33),P=0.09,the incidence of adverse reactions(RR=0.35,95%CI 0.23~0.53,P<0.00001).[Conclusion]compared with single triple therapy,the combination of Banxia Xiexin decoction can effectively improve the efficacy and safety of clinical treatment of Hp related PU.However,due to the quality of the included studies,the conclusion of this study needs to be further verified by more high-quality RCT tests.
