原发性肝癌中CD133和EpCAM蛋白表达与阿帕替尼疗效的关系研究

摘　　要：目的探讨阿帕替尼一线治疗晚期原发性肝癌的有效性,分析其疗效的影响因素,寻找有效的疗效预测标志物。方法回顾性分析华中科技大学同济医学院附属协和医院一线使用阿帕替尼的晚期原发性肝癌患者33例,取得肝癌组织切片,免疫组化法检测肝癌干细胞标志物CD133、CD90和EpCAM的表达水平情况,分析其表达对阿帕替尼疗效的影响。结果33例患者无进展生存期(PFS)为(2.8±0.3)个月,3个月、4个月、5个月的疾病控制率分别为60.3%、52.3%、42.1%。卡方检验显示,肝癌组织中CD133和EpCAM的表达与阿帕替尼疗效呈负相关。结论阿帕替尼是治疗晚期原发性肝癌有效手段之一,肝癌组织中CD133和EpCAM的表达可用于预测阿帕替尼疗效。Objective To investigate the efficacy of Apatinib in first-line treatment of advanced primary liver cancer,analyze the influencing factors of efficacy,and search for effective biomarkers for efficacy prediction.Methods A retrospective analysis was performed on 33 patients with advanced primary liver cancer who were treated with Apatinib in Union Hospital,Tongji Medical College,Huazhong University of Science and Technology.Liver cancer tissue sections were obtained,and the expressions of liver cancer stem cell markers CD133,CD90 and EpCAM were detected by immunohistochemistry,to analyze the effect of the expressions of the three on the efficacy of Apatinib.Results The PFS of 33 patients was(2.8±0.3)months,and the disease control rate at 3 months,4 months and 5 months was 60.3%,52.3%and 42.1%,respectively.The Chi-square test indicated that the expressions of CD133 and EpCAM in liver cancer tissues was negatively correlated with the efficacy of Apatinib.Conclusion Apatinib is one of the effective methods to treat patients with advanced primary liver cancer.Expression of CD133 and EpCAM in liver cancer tissues can be used to predict the efficacy of Apatinib.
