肝硬化食管胃底静脉曲张出血的胃镜治疗效果分析

摘　　要：目的观察肝硬化食管胃底静脉曲张破裂出血(EGVB)患者入院后,在不同时间段内进行内镜下治疗的效果分析。方法选取108例肝硬化EGVB患者资料进行回顾性分析,根据胃镜治疗时间段的不同分为急诊组和常规组。比较2组患者入院后的住院天数、费用、再出血率、并发症发生率及病死率。比较腹痛、恶心、便潜血消失时间。结果急诊组患者的总费用、住院天数均明显短于常规组(P<0.01)。急诊组患者的再出血、并发症、病死率发生率低(P<0.05)。急诊组患者腹痛、恶心、便潜血消失时间均短于常规组(P<0.01)。结论肝硬化EGVB患者,急诊胃镜治疗可以提高疗效,减轻患者费用,缩短住院时间,继而降低再出血率,甚至病死率,临床症状消失时间短,疗效满意度高。Objective To determine the clinical effect of gastroscopy in hepatic cirrhosis patients with esophageal and gastric varices bleeding(EGVB)in different time frame.Methods The clinical data of 108 hepatic cirrhosis patients with EGVB were retrospectively analyzed.According to the time frame of gastroscopy,all patients were divided into emergency group and routine group.The length of hospital stay,total cost,rebleeding rate,incidence of complication,and mortality were compared between the two groups.The time to no symptoms,such as abdominal pain,nausea and occult blood in stool were compared between the two groups.Results The total cost and length of hospital stay in the emergency group were significantly shorter than those in the routine group(P<0.01).The rebleeding rate,incidence of complication and mortality in the emergency group was low(P<0.05).The time to no symptoms,such as abdominal pain,nausea and occult blood in stool in the emergency group was shorter than that in the routine group(P<0.01).Conclusion According to the retrospective study,using emergency gastroscopy in hepatic cirrhosis patients with EGVB may improve the efficacy,reduce patients′costs,shorten the length of hospital stay,reduce the rebleeding rate and even mortality.The time to no clinical symptoms is short,and the efficacy satisfaction is high.
