藏汉人群血清胃功能三项联合Hp检测在胃癌筛查中的临床研究解析

摘　　要：目的:分析血清胃功能三项联合Hp检测在胃癌筛查中的作用,并分析藏族及汉族人群胃癌发生率的差异性。方法:选取2018年5月-2020年5月进行胃癌筛查的藏族和汉族各100例,所有患者均进行血清胃功能三项(胃泌素-17、胃蛋白酶原Ⅰ、胃蛋白酶原Ⅱ)联合幽门螺杆菌(Hp)检查,并行胃镜检查,分析检查情况。结果:胃癌患者血清胃泌素-17、胃蛋白酶原Ⅱ水平明显升高,Hp阳性率高;藏族人群的胃癌发病率高于汉族人群,差异有统计学意义(P<0.05)。结论:血清胃泌素-17、胃蛋白酶原Ⅱ水平升高以及Hp感染提示有胃癌发生的高风险,因此Hp联合血清胃功能三项可辅助筛查胃癌,藏族人群胃癌发生率高于汉族人群,后续需要注意加强饮食管理及胃癌筛查。Objective:To study and analyze the effect of serum gastric function combined with Hp detection in gastric cancer screening and to analyze the difference in the incidence rate of gastric cancer between Tibetan and Han populations.Methods:From May 2018 to May 2020,100 cases of Tibetan and Han nationals who were screened for gastric cancer were enrolled,all patients were examined for three items of serum gastric function(gastrin-17,pepsinogen Ⅰ,pepsinogen Ⅱ)in combination with Helicobacter pylori(Hp)examination,gastroscopy was performed to analyze the examination.Results:Serum gastrin-17 and pepsinogen Ⅱ levels were significantly increased in gastric cancer patients,the Hp positive rate was high.The incidence rate of gastric cancer in Tibetan population was higher than that in Han population,the difference was statistically significant(P<0.05).Conclusion:Elevated levels of serum gastrin-17,pepsinogen Ⅱ and Hp infection suggest a high risk of gastric cancer,therefore,Hp with serum gastric function can be used to screen gastric cancer.The incidence rate of gastric cancer in Tibetan people is higher than that in Han people.Attention should be paid to strengthen diet management and gastric cancer screening regularly.
