胃镜下组织胶注射治疗肝硬化并发胃底静脉曲张破裂出血患者的临床效果

摘　　要：目的探讨胃镜下组织胶注射治疗肝硬化并发胃底静脉曲张破裂出血患者的临床效果。方法选取2016年9月~2019年3月于高州市人民医院治疗的62例肝硬化并发胃底静脉曲张破裂出血的患者作为研究对象,按随机数字表法将其分为对照组和观察组,每组各31例。对照组行胃镜下套扎术治疗,观察组行组织胶注射治疗。比较两组的肝硬化并发胃底静脉曲张改善的总有效率,比较两组的血红蛋白(Hb)、红细胞(RBC)、总胆红素(TBil)、清蛋白(ALB)水平及不良反应发生情况。结果治疗后,观察组的总有效率为96.77%,高于对照组的77.41%,差异有统计学意义(P<0.05);观察组的Hb、RBC、ALB水平高于对照组,TBil水平低于对照组,差异有统计学意义(P<0.05);观察组的不良反应总发生率为12.90%,低于对照组的38.71%,差异有统计学意义(P<0.05)。结论胃镜下组织胶注射治疗肝硬化并发胃底静脉曲张出血患者,能有效改善其静脉曲张情况,止血效果好,且能减少不良反应的发生。Objective To investigate the clinical effect of gastroscopic Tissue Adhesive injection in the treatment of patients with cirrhosis and variceal bleeding.Methods A total of 62 patients with cirrhosis complicated with gastric varicose bleeding who were treated in Gaozhou People's Hospital from September 2016 to March 2019 were selected as the research objects.They were divided into a control group and an observation group according to the random number table method,with 31 cases in each group.The control group underwent endoscopic ligation,and the observation group received Tissue Adhesive injection therapy.The total effective rates of improvement of gastric varices combined with improvement of gastric varices of the two groups were compared,and the hemoglobin(Hb),red blood cells(RBC),total bilirubin(TBil),albumin(ALB)levels and adverse reactions of the two groups were compared.Results After treatment,the total effective rate of the observation group was 96.77%,which was higher than that of the control group of 77.41%,the difference was statistically significant(P<0.05).The Hb,RBC,ALB levels of the observation group were higher than those in the control group,TBil level of the observation group was lower than that in the control group,the differences were statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was 12.90%,which was lower than that in the control group of 38.71%,the difference was statistically significant(P<0.05).Conclusion Gastroscopic Tissue Adhesive injection for patients with cirrhosis and gastric varices bleeding can effectively improve varicose veins,good hemostasis and can reduce the incidence of adverse reactions.
