摘  要:目的比较青海海南地区藏、汉族人群颈动脉斑块与血浆同型半胱氨酸(Hcy)、高敏C反应蛋白(hs-CRP)的关系。方法﹑采用随机抽样方法筛查常住海拔3000米左右脑卒中危险人群(318例),用彩超测量颈总动脉内中膜厚度(IMT)。根据测量结果,分为藏、汉族颈动脉斑块阳性组和阴性组。收集研究对象的临床资料,采集血样冷冻,利用循环酶法、散射比浊法分别测定血浆Hcy和hs-CRP水平。分析青海海南地区藏、汉族人群颈动脉斑块检出情况及其与血浆Hey,hs-CRP的关系和民族间差异。结果﹑藏族颈动脉斑块检出率为26.9%,高于汉族17.0%(χ^2=4.462,P<0.05);藏、汉族颈动脉斑块阳性组血浆Hcy、hs-CRP水平高于阴性组(16.7±4.2 vs 11.0±4.0,5.3±2.2 vs 2.3±1.9;12.8±3.10s 9.1±2.9,6.5±2.4 vs 3.7±2.1。P均<0.01);藏族血浆Hcy水平高于汉族(P<0.01),而血浆hs-CRP水平低于汉族(P<0.01)。多因素Logistic回归分析显示,IMT、Hcy,hs-CRP水平是颈动脉斑块形成的危险因素。结论青海海南地区藏族颈动脉硬化发生率高于汉族,可能与藏族血浆Hcy水平高于汉族相关;藏族hs-CRP水平低于汉族。Objective To compare the relationship between carotid artery plaque,plasma homocysteine(Hcy)and hypersensitive C reactive protein(hs-CRP)in Tibetan and Han nationalities in Hainan area of Qinghai province.Method 318 patients living at an altitude of 3000 meters with stroke risk were screened by random sampling method.The intima-media thickness(IMT)of common carotid artery was measured by color Doppler ultrasound.According tothe results of measurement,the patients were divided into positive group and negative group of carotid artery plaque inTibetan and Han nationalities.The clinical data of the subjects were collected and blood samples were frozen.The lev-els of Hcy and hs-CRP in plasma were determined by Enzymatic cyeling assay and Nephelometry so as to analyze the detection of carotid artery plaque and its relationship with Hey and hs-CRP in plasma as well as ethnic differences inTibetan and Han nationalities in Hainan area of Qinghai province.Results The detection rate of carotid artery plaquein Tibetan nationality was 26.9%higher than that in Han nationality of 17.9%(χ^2=4.462,P<0.05).The levels ofHcy and hs-CRP in plasma in positive group of carotid artery plaque in Tibetan and Han nationalities were higherthan those in negative group(16.7±4.20s 11.014.0,5.3±2.2 vs 2.3±1.9;12.8±3.10s 9.1±2.9,6.5±2.4 s 3.7±.2.1,P<0.01);compared with Han nationality,the level of Hcyin plasma in Tibetan nationality was higher(P<0.01),while the level of hs-CRP in plasma in Tibetan nationality was lower(P<0.01).Mulivariate logistic regression analy-sis showed that the levels of IMT,Hcy and hs-CRP were rsk factors for the formation of carotid artery plaque.Conclusions The incidence of carotid atherasclerosis in Tibetan nationality is higher than that in Han nationality in Hain-an area of Qinghai province,which may be related to the higher level of Hcy in plasma in Tibetan nationality;the levelof hs-CRP in Tibetan nationality is lower than that in Han nationality.
