中医药不同治则对非小细胞肺外周血免疫指标影响的临床研究

摘　　要：目的通过研究扶正(益气养阴方)、祛邪(清热解毒方)、扶正祛邪(益气养阴解毒方)3种治则对非小细胞肺癌患者自然杀伤细胞、T细胞亚群、B细胞等免疫指标的作用,探究不同治则对非小细胞肺癌患者的免疫功能的影响。方法采用完全随机、平行对照的实验设计,将166例非小细胞肺癌患者按照随机数字表纳入研究,完成益气养阴方组(A组)39例、清热解毒方组(B组)46例、益气养阴解毒方组(C组)49例,分别给予相应的中药汤剂,4周为1个疗程,治疗3个疗程(12周)后观察对外周血免疫指标影响。结果非小细胞肺癌患者治疗前外周血CD3、CD8水平高于健康人群组,差异有统计学意义(P<0.001),而CD4、CD4/CD8、CD3-CD16/56+水平明显低于健康人群组,差异具有统计学意义(P<0.001)。治疗后,A组和C组能够明显降低非小细胞肺癌患者外周血CD8水平(P<0.05),两组之间比较无显著性差异,提示益气养阴方与益气养阴解毒方可通过降低CD8来调节机体免疫功能。A、B、C 3组对CD3、CD4、CD4/CD8、CD19、NK细胞均无明显的调节作用(P>0.05)。结论扶正和扶正祛邪治则均可调节肺癌患者的免疫功能,从而预防肿瘤复发与转移。Objective To investigate the effects of three therapeutic principles on immune indexes of non-small cell lung cancer(NSCLC) patients,including natural killer cells,T cell subsets and B cells,andexplore the effects of different therapeutic principles on immune function of NSCLC patients by studying the effects of three therapeutic principles:tonifying vital Qi(Yiqi Yangyin Formula),dispelling evils(Qingre Jiedu Formula) and :tonifying vital Qi and dispelling evils(Yiqi Yangyin Jiedu Formula).Methods A total of 166 patients with NSCLC were enrolled in the study according to random number table.Group A(39 cases),group B(46 cases) and group C(49 cases) were treated with Chinese herbal decoction respectively.Four weeks were a course of treatment,and we observed the influence of immune indexes in peripheral blood after three courses of treatment(12 weeks).Results The levels of CD3 and CD8 in peripheral blood of NSCLC patients before treatment were higher than those of healthy people(P<0.001),while the levels of CD4,CD4/CD8 and CD3-CD16/56+ were significantly lower than those of healthy people(P<0.001).After treatment,group A and group C could significantly reduce the level of CD8 in peripheral blood of patients with NSCLC(P<0.05).There was no significant difference between the two groups,suggesting that Yiqi Yangyin Formula and Yiqi Yangyin Jiedu Formula could regulate the immune function by reducing CD8.There were no significant regulatory effects on CD3,CD4,CD4/CD8,CD19 or NK cells in group A,B and C(P>0.05).Conclusion Both tonifying vital Qimethod and tonifying vital Qi and dispelling evils method can regulate the immune function of lung cancer patients and prevent the recurrence and metastasis of cancer.
