行气解郁方对脑卒中后抑郁患者神经功能及炎症因子水平的影响

摘　　要：目的探究行气解郁方对脑卒中后抑郁(PSD)患者神经功能及炎症因子水平的影响。方法选取2014年9月—2016年12月在该科住院及门诊治疗的100例PSD患者,随机将其分为中药治疗组(50例)和西药对照组(50例)。两组PSD患者均给予常规脑卒中药物及康复治疗。西药对照组给予氟哌噻吨美利曲辛片,中药治疗组予中药行气解郁方。治疗7周,观察并比较两组的汉密尔顿抑郁量表(HAMD)评分、美国国立卫生研究院卒中量表(NIHSS)评分、炎症因子及疗效。结果治疗后,两组HAMD得分,NIHSS得分,白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平均显著低于治疗前(P<0.05),且中药治疗组HAMD得分,NIHSS得分,IL-6、IL-8、TNF-α水平显著低于西药对照组(P<0.05)。中药治疗组总有效率显著高于西药对照组(96.00%,48/50 vs. 80.00%,40/50,P<0.05)。结论行气解郁方能缓解卒中后抑郁患者的抑郁程度,提高生活能力,可显著改善其血清炎症因子异常分泌情况、神经功能,值得推广。Objective To explore the effect on neurological function and inflammatory factors in patients with post-stroke depression(PSD) by using Xingqi Jieyu Recipe.Methods A total of 100 patients with PSD hospitalized in the inpatient department of our hospital′s rehabilitation department between September 2014 and December 2016 were randomly divided into the western medicine control group(50 cases) and the Chinese medicine treatment group(50 cases).Both groups were given conventional stroke drugs and rehabilitation treatment.The western medicine control group was given Flupentixol Melitracen Tablets while the Chinese medicine treatment group was given Chinese medicine Xingqi Jieyu Recipe.After 7 weeks of treatment,scores of Hamilton Depression Scale(HAMD) and National Institutes of Health Stroke Scale(NIHSS),inflammatory factors and efficacy were observed and compared between the two groups.Results After treatment,the scores of HAMD and NIHSS,levels of interleukin-6(IL-6),interleukin-8(IL-8) and tumor necrosis factor-α(TNF-α) in the two groups were significantly lower than those before treatment(P<0.05),and the scores of HAMD and NIHSS,levels of IL-6,IL-8 and TNF-α in the Chinese medicine treatment group were significantly lower than those in the western medicine control group(P<0.05).The total effective rate in the Chinese medicine treatment group was significantly higher than that in the western medicine control group(96.00%,48/50 vs. 80.00%,40/50,P<0.05).Conclusion Xingqi Jieyu Recipe combined with Flupentixol Melitracen Tablets has good clinical effect on patients with PSD,which can significantly improve the abnormal secretion of serum inflammatory factors,neurological function and related clinical symptoms,and is worthy of promotion.
