宫颈环形电切术+重组干扰素α-2b凝胶治疗宫颈上皮内瘤变伴高危型人乳头瘤病毒感染的临床评价

摘　　要：目的探究宫颈环形电切术(LEEP)+重组干扰素α-2b凝胶治疗宫颈上皮内瘤变(CIN)伴高危型人乳头瘤病毒(HR-HPV)感染的临床效果,为临床医师选择合理治疗方案提供参考。方法 82例CIN伴HR-HPV感染患者为研究对象,依据随机数字表法将患者分为A组和B组,每组41例。B组患者接受LEEP治疗, A组采用LEEP+重组干扰素α-2b凝胶治疗。比较两组临床治疗效果、复发率、HPV清除率、HPV病毒载量。结果 A组临床治疗总有效率为95.12%,高于B组的80.49%,差异有统计学意义(P<0.05)。A组复发率为2.44%,低于B组的14.63%;A组HPV清除率为92.68%,高于B组的75.61%,差异均有统计学意义(P<0.05)。治疗后,两组HPV病毒载量均少于治疗前,且A组(1.37±0.57)×106 copies/ml少于B组的(2.03±0.42)×106 copies/ml,差异有统计学意义(P<0.05)。结论宫颈环形电切术+重组干扰素α-2b凝胶治疗宫颈上皮内瘤变伴高危型人乳头瘤病毒感染的临床效果显著,能够促进患者病情恢复,提高病毒清除率,降低病毒载量,值得推荐。Objective To investigate the clinical effect of loop electrosurgical excision procedure(LEEP) combined with recombinant interferonα-2 b gel in the treatment of cervical intraepithelial neoplasia(CIN)with high-risk human papillomavirus infection(HR-HPV),and provide reference for rational selection of regimen.Methods A total of 82 patients with CIN and HR-HPV infection as study subjects were divided into group A and group B according to random numerical table,with 41 cases in each group.Group B was treated with LEEP,and group A was treated with LEEP combined with recombinant interferonα-2 b gel.The clinical effect,recurrence rate,HPV clearance rate and HPV viral load were compared between the two groups.Results The total effective rate of clinical treatment of group A was 95.12%,which was higher than 80.49%of group B,and the difference was statistically significant(P<0.05).The recurrence rate of group A was 2.44%,which was lower than 14.63%of group B;HPV clearance rate of group A was 92.68%,which was higher than 75.61%of group B.The difference was statistically significant(P<0.05).After treatment,HPV viral load of the two groups were less than those before treatment,and group A(1.37±0.57)×106 copies/ml was less than that of group B(2.03±0.42)×10~6 copies/ml,and the difference was statistically significant(P<0.05).Conclusion Loop electrosurgical excision procedure combined with recombinant interferonα-2 b gel has significant clinical effects in the treatment of cervical intraepithelial neoplasia with high-risk human papillomavirus infection,which can promote the recovery of patients,increase virus clearance rate,and reduce viral load.It is worthy of recommendation.
