超声引导腕管注射类固醇治疗腕管综合征及其对神经电生理的影响

摘　　要：目的观察超声下腕管注射类固醇治疗腕管综合征(CTS)疗效及对神经电生理指标的影响。方法在超声引导下对102例CTS患者(120只患腕)单次腕管注射曲安奈德和利多卡因混合液,观察其1个月疗效、症状严重程度评分(SSS)、功能状态评分(FSS)及神经电生理指标。结果治疗后1个月随访,102例CTS患者(120患腕)中优20腕(16.7%),良76腕(63.3%),一般18腕(15.0%),差6腕(5.0%);治疗后SSS、FSS评分及运动末端潜伏期均明显低于治疗前(P<0.05)。结论超声引导下腕管注射类固醇治疗CTS简单、安全,短期效果确切,并改善部分神经电生理指标。Objective To observe the therapeutic and neuroelectrophysiological effect of ultrasound-guided carpal tunnel injection of steroid on carpal tunnel syndrome(CTS).Methods A total of 102 CTS patients(120 wrists)were treated with single ultrasound-guided carpal tunnel injection of triamcinolone and lidocaine mixture.Clinical efficacy,symptom severity score(SSS),functional status score(FSS),and neuroelectrophysiological indexes were evaluated 1 month posttreatment.Results Of 120 wrists,there were excellent in 20(16.7%),good in 76(63.3%),fair in 18(15.0%),and poor in 6(5.0%)wrists at 1-month follow-up.SSS,FSS and distal motor latency were decreased posttreatment compared with pretreatment.Conclusion Ultrasound-guided carpal tunnel injection of steroid is simple and safe for CTS with satisfactory short-term effect and neuroelectrophysiological improvement.
