摘　　要：目的探讨能量合剂联合糖皮质激素对晚期肿瘤合并营养不良患者癌因性疲乏(CRF)的影响。方法选取2017年3月至2018年3月邯郸市中心医院收治的150例晚期肿瘤合并营养不良的患者为研究对象,采用计算机随机数字法将其分为观察组和对照组各75例。两组患者均已完成放化疗超过30 d,对照组予以对症支持治疗,观察组在对照组基础上予以能量合剂联合糖皮质激素治疗。观察两组患者治疗前后血红蛋白(Hb)、Karnofsky功能状态评分(KPS)、癌症疲乏自评量表(PFS)、癌症生活质量测定量表(EORTC QLQ-C30)等指标的变化情况。结果治疗6周后,两组患者Hb值、KPS评分、EORTC QLQ-C30各维度及总生活质量评分均较治疗前升高,且观察组高于对照组(P<0.05),两组患者PFS各维度得分及疲乏分值均较治疗前降低,且观察组低于对照组(P<0.05)。结论能量合剂联合糖皮质激素可有效改善晚期肿瘤合并营养不良患者的营养和健康状态,减少癌因性疲乏并提高生活质量。Objective To explore the effects of energy mixture combined with glucocorticoid on cancer-related fatigue(CRF)in patients with advanced tumor complicated with malnutrition.Methods A total of 150 patients with advanced tumor and malnutrition admitted to the Handan Central Hospital from March 2017 to March 2018 were selected and divided into observation group and control group by the method of random number table,with 75 cases in each group.The patients in two groups were treated with radiotherapy and chemotherapy for more than 30 d.And the patients in control group were given symptomatic supportive therapy who were compared with the patients in observation group treated with energy mixture combined with glucocorticoid therapy on the basis of control group.The changes of hemoglobin(Hb),Karnofsky functional status score(KPS),Piper fatigue self-rating scale(PFS)and cancer quality of life measurement scale(EORTC QLQ-C30)were observed in the two groups before and after treatment.Results After 6 weeks of treatment,the Hb values,KPS scores,dimensions scores and total quality of life scores of EORTC QLQ-C30 in the two groups were higher than those before treatment.And the indexes in observation group were higher than those in control group(P<0.05).The dimensions scores of PFS and the scores of fatigue in the two groups were lower than those before treatment.And the scores in observation group were lower than those in control group(P<0.05).Conclusions Energy mixture combined with glucocorticoid can effectively improve the conditions of nutrition and health of patients with advanced tumor and malnutrition,and reduce cancer-related fatigue,which improves the quality of life.
